Request Guidelines for Community Event and Activity Sponsorships

Kaiser Permanente San Francisco has two programs that support community events and activities through financial sponsorship, in-kind donations, and staff involvement:

 Community Benefit

 Community Relations

We will accept only one request to either Community Benefit or Community Relations during the calendar year, based on organization’s/project’s match with each program’s priority areas. Requests must be received a minimum of three months before the date of the event.

Community Benefit Program

The KP-SF Community Benefit Program supports local, health-focused, nonprofit organizations whose mission and services to vulnerable San Francisco residents align closely with Kaiser Permanente San Francisco’s community health priority areas:

1. Increase Access to High Quality Health Care and Services

2. Increase Healthy Eating and Active Living (HEAL)

3. Ensure Safe and Healthy Living Environments

How to Apply for Community Benefit Sponsorship

Organizations that receive a KP-SF Community Benefit grant will not be considered for sponsorship funding in the same calendar year in which they are providing grant-funded services. To be considered for sponsorship funding for events or activities, the services your organization provides must address at least one of the health needs identified above. Please submit your request on your agency’s letterhead and include the following information:  Organization name and short description of your mission/purpose

 The Community Benefit priority area that your organization matches and how your services are addressing this health need.

 The geographic area served by your organization, the number of people served annually, and the San Francisco target populations served.

 History of partnering with Kaiser Permanente – for the past 5 years, the year and amount of grants, sponsorships, donations, and service of KP employees as board members.

 Short description of your event with date, time, and location

 Proposed sponsorship level, and how the requested funding will be used.

 Contact name, phone number and email address.

Remember to include an electronic copy of:

1. Request letter, including all the information above.

2. Event announcement (if any)

3. Sponsorship levels with associated benefits.

4. Your organizations IRS tax status determination letter.

5. List of Board of Directors with affiliations.

6. Organization’s nondiscrimination policy on your letterhead.

Community Relations Program

Community Relations supports local community events and activities which reflect Kaiser Permanente’s mission and cause, through sponsorships, in-kind donations, and volunteer efforts:

1. Healthy Lifestyles: Supporting events and initiatives that contribute to the overall health and well-being of individuals and communities.

2. Education: Working in partnership with organizations that create positive educational environments in the San Francisco Unified School District.

3. Workforce Development: Providing opportunities and supporting programs to foster interest in health care fields. Event sponsorships are by invitation only.
