ASHLAND BOARD OF EDUCATION REGULAR MEETING Monday, May 23, 2016 5:30 P.M.

The Ashland Board of Education met in regular session on May 23, 2016, at the board office of 1820 Hickman Street, Ashland, KY 41101. Chairperson Carol Jackson called the meeting to order.

Order # 83 Upon the recommendation of the superintendent, it was moved by Minutes Frank DeMartino and seconded by David Latherow to approve the minutes of the special meeting of May 5, 2016 and the regular meeting of April 25, 2016.

CIO Cary Williams addressed the board regarding this year’s STLP and Robotics program. Coaches from each school introduced attending students and announced their accomplishments this year. Samantha Royalty from Oakview Elementary introduced students who were selected as Level 2 finalists for their project “How to @ Oakview.” Claudia Lovin from Charles Russell introduced students on the first place Minecraft team as well as students who served as Scratch Ambassadors. Amanda Cole from Crabbe Elementary introduced the students who were on the 2nd place robotics team. Mrs. Williams mentioned additional winners from Ashland Middle School and Paul G. Blazer High School who were not able to attend the board meeting.

Instructional Supervisor Linda Calhoun introduced Lori Baier, Blazer Health and Science teacher and senior students from the CMNA and Phlebotomy career pathways. The CMNA program is in the second year of implementation. This is the first year of the Phlebotomy pathway. Nine students successfully completed state certification for CMNA. Twelve students successfully completed state certification for Phlebotomy. Of these seniors, nine have dual certification. Five of our seniors are Rural Health Scholars and one junior will participate in the program next year.

Jamie Fannin, with KDMC presented their My Health Solutions presentation to the board. My Health Solutions provides walk-in care for students and staff, occupational medicine, health tools and analytics, and mobile preventive health services. They provide video- based telemedicine and remote monitoring, on-site nurse practitioners or physician assistant clinic at worksites, staff safety training, and group education programs.

Greg Jackson spoke on the Putnam Donor Wall and brick placement project. He requested board approval for bidding the next phase.

Order # 84 Upon the recommendation of the superintendent, it was moved by Amend Chris Clarke and seconded by Frank DeMartino to amend the agenda.

Order # 85 Upon the recommendation of the superintendent, it was moved by Donor Wall Frank DeMartino and seconded by David Latherow to approve the Plans and allow for bid the next phase for the Putnam Donor Wall. Ayes: Clarke, DeMartino, Latherow, Jackson

Maria Gillette, Murphy Graves Trimble updated the board on the AMS, Blazer and Alumni Gym projects.

Teresa Conway, Director of Head start presented the following to the board: Health Services & Nutrition Plan Early Childhood Development and Health Services Plan Program Design and Management Plan ERSEA Plan Transportation Plan Community Assessment Updates TS Gold 2015-16 Spring Assessment Report Certification of Governance and Leadership Capacity Screening Grant Duration Grant

Order # 86 Upon the recommendation of the superintendent, it was moved by KEDC Membership Chris Clarke and seconded by David Latherow to approve the 2016- 2017 KEDC membership in the amount of $1,997.79.

Order # 86 Upon the recommendation of the superintendent, it was moved by KSBA E-Meeting David Latherow and seconded by Frank DeMartino to approve the Agreement to Subscribe to the KSBA E-Meeting Service.

Finance Director, Tim Walters presented the 2016-2017 tentative budget.

Order # 87 Upon the recommendation of the superintendent, it was moved by Tentative Budget Frank DeMartino and seconded by David Latherow to approve the 2016-2017 tentative budget.

Order # 88 Upon the recommendation of the superintendent, it was moved by 2016-17 Auditor Frank DeMartino and seconded by David Latherow to approve Kelley, Galloway and Company as the auditor for the 2016-2017 school year.

Order # 89 Upon the recommendation of the superintendent, it was moved by Fidelity Bond Chris Clarke and seconded by Frank DeMartino to approve the Fidelity Bond for the Business Manager/Treasurer for the 2016-2017 school year.

Order # 90 Upon the recommendation of the superintendent, it was moved by 2016-17 Salary Frank DeMartino and seconded by Chris Clarke to combine and Schedules approve the following:  2016-2017 Certified Salary Schedule  2016-2017 Classified Salary Schedule (Employees hired prior to August 1, 1998).  2016-2017 Classified Salary Schedule (Employees hired after August 1, 1998).

Ayes: Clarke, DeMartino, Latherow, Jackson Order # 91 Upon the recommendation of the superintendent, it was moved by Consent David Latherow and seconded by Frank DeMartino to combine and approve the following consent items:

 Bills for May, 2016 as presented by Board Treasurer.  Head Start bills for May, 2016 as presented by the Board Treasurer.  Salaries for May, 2016 in accordance with the 2015-2016 salary schedule.  Head Start salaries for May, 2016 in accordance with the 2015- 2016 salary schedule.

Order # 92 Upon the recommendation of the superintendent, it was moved by Rescind David Latherow and seconded by Frank DeMartino to approve Policy 09.12 rescinding the amendment of Policy 09.12 “Admissions and Amendment Attendance” to insert wording regarding discipline and attendance of nonresident students adopted at the meeting on April 25, 2016.

Order # 93 Upon the recommendation of the superintendent, it was moved by Rescind Frank DeMartino and seconded by Chris Clarke to approve rescinding Procedure 09.12 the amendment to Procedure 09.12 AP.21 “Application for Nonresident AP.21 Student Registration Form” to include wording regarding discipline and Amendment attendance of nonresident students adopted at the meeting on April 25, 2016.

Order # 94 Upon the recommendation of the superintendent, it was moved by MOA Morehead Frank DeMartino and seconded by Chris Clarke to approve the Dual Credit Memorandum of Agreement between Morehead State University and Ashland Independent Schools for the dual credit program.

Order # 95 Upon the recommendation of the superintendent, it was moved by Amend David Latherow and seconded by Frank DeMartino to amend the agenda to include the approval of the following Head Start documents:  Community Assessment Updates  TS Gold 2015-2016 Spring Assessment Report  Grant  Duration Grant

Order # 96 Upon the recommendation of the superintendent, it was moved by Head Start Plans Frank DeMartino and seconded by Chris Clarke to approve the following Head Start plans:

 Health Services & Nutrition Plan  Early Childhood Development and Health Services Plan  Program Design and Management Plan  ERSEA Plan  Transportation Plan.

Ayes: Clarke, DeMartino, Latherow, Jackson Order # 97 Upon the recommendation of the superintendent, it was moved by MOU Pathways Chris Clarke and seconded by David Latherow to approve the Memorandum of Understanding with Pathways, Inc. for the 2016-2017 school year.

Order # 98 Upon the recommendation of the superintendent, it was moved to combine and approve the following items:

 Orientation and Mobility Contract with Karen Hinkle for the 2016- 2017 school year.  Contract with Grayson Speech Therapy Services, P.S.C. to provide physical and occupational therapy services for the 2016-2017 school year.  Contract with East Kentucky Psychological Resources for psychological evaluations for the 2016-2017 school year.

Order # 99 Upon the recommendation of the superintendent, it was moved New Business by David Latherow and seconded by Frank DeMartino to combine and Consent approve the following items:

 Fundraiser requests as listed: Ashland Middle School Cheerleading Tri-State Fundraising card sales, car wash, Krispy Kreme sales, mum sale to raise funds for equipment and competition fees; Chain Links after-school candy sales to raise money to help support Cash and Coins for Christmas, collect donations at end of October and beginning of November to help benefit the VA Hospital; Discovery Team Scragglepop, McTeacher Night, Zanzi’s pizza sales to raise funds for team activities.

 Use of Blazer High School’s gymnasium by the girls’ basketball team for a basketball camp on June 6 and 7, 2016.
