Carol Robidoux JP Services Couples Questionnaire

Greetings, and congratulations on your upcoming wedding day. This step is important in that it helps me to really personalize your ceremony. The more you can tell me, the more I will have to work with. My goal is that you and your guests come away from the service feeling like it truly was a reflection of you both, and your relationship. For wedding 911 emergencies, or just to check in, my cell phone number is 603-930-0640.

Carol Robidoux, JP [email protected]

Your full legal name: Home Address: Phone (Best way to reach you): Birthdate: Occupation: Name to be used in ceremony (if you prefer a nickname such as Rob instead of Robert, etc.) Wedding details (place, date, time):

Wedding colors, themes, songs you will be using for entrance/exit:

Rehearsal details: (date, time) if applicable:

Brief synopsis of how you met:

Three qualities you most admire in your future spouse (feel free to elaborate):

When and how did you know this was the person you wanted to commit to?

What's the hardest thing you've had to work through so far in your relationship?

What do you anticipate may be your biggest hurdles in life together?

Is there a couple you admire, and if so, what do you most admire about their relationship? Do you have a favorite “dating” story that you think sums up the spirit of your relationship?

Name three interesting facts about you that not everybody knows (or individual/shared hobbies activities you enjoy):

As for your vows, which of the following would you be most comfortable including? (Rate them numerically):

__ Biblical or other religious or spiritual text (state your particular preference here if you have one). __ Composing and exchanging individual vows or promises. __ Poetry or music (include a favorite poem, lyric or poet here). __ Straightforward traditional vows to which you simply answer “I do” or “I will”

Have you considered having family or friends participate by reading or speaking briefly during the ceremony? For example, some couples have had their mother’s both read poetry, or had siblings with skills sing a special song at some point. If this is something you might be interested, let me know. I may be able to help you select appropriate readings.

Go for it: Your chance to tell me what your expectations are for your ceremony, in terms of tone, text, content, etc. Also, if there are any other symbolic elements or traditions you would like to include (Unity candle; sand mixing; hand fasting; broom jumping; glass breaking; butterfly releasing; square dancing – well, ok; just kidding about that last one, but you get the idea):

Reminders  Make sure you have secured your marriage license in the city or town where you will be wed in advance of the ceremony. Bring that paperwork with you to the rehearsal. I’ll take it from there. If you have any questions, I have answers.  Payment for JP services are appreciated in advance of the ceremony. You may make a deposit to hold the date, but make sure your balance is paid in full two weeks before your wedding date. Thank you very much for selecting me as your JP. I am looking forward to your big day – almost as much as you are.
