Institutional Review Board (IRB) Forms

Letter of Intent to Serve as Agency Preceptor
The Special Project is one option for the culminating academic experience, required in the Master of Public Health program in the SCSU Department of Public Health. To complete an agency- based Special Project a graduate student will work with an agency preceptor to respond to a void or gap in service. The Special Project must meet the Special Project Guidelines and Requirements published by the Department of Public Health. The Special Project is completed under the guidance and supervision of a faculty member of the Department and an Agency Preceptor. The Special Project is composed of two documents: a) the Special Project Prospectus, which is a proposal to provide a service to respond to the void or gap in service, and b) the Special Project Report, which describes the process for the development of the response to the void or gap in service.
The student's work is monitored by the Special Project Advisor and reviewed periodically by the Agency Preceptor. The final Special Project Report is approved by the Project Advisor and Agency Preceptor.
The Agency Preceptor’s responsibilities include, but may not be limited to, the following:
1. Approve the concept for the Special Project with special attention to the needs of the agency. 2. Provide students with access to all relevant resources for completing the Special Project. 3. Provide guidance to the student and review drafts of the Prospectus and Report, as necessary. 4. Approve the Special Project, if it meets the requirements agreed upon by the student, Advisor and Agency Preceptor.
The Agency Preceptor is encouraged to contact the student's Project Advisor at any time.

Over half of undergraduate college women in the U.S. have revealed at least one experience with sexual coercion during their college years. Approximately 12% of college women were victims of forcible rape in 1997. Healthy People 2010 proposes to reduce the rate of forced sexual intercourse or attempted intercourse of persons 12 and older to less than 0.55 per 1,000. According to the experts, the most effective means of preventing rape on college campuses is education. Many current programs place emphasis on the potential victim in contrast to the potential assailant. The goal of this project was to raise the awareness about sexual assault and its consequences among male college students by developing an educational innovation that addressed the predisposing, enabling and reinforcing factors related to sexual assault. The cost of the Project was estimated to be $38, 256.
