Job Title: Penrose Community Links – Co-ordinator

Reports to: Senior Management

Location: Based in Islington but covering Lambeth and Camden

Salary: £22,865

Hours: 37.5 per week

Job Purpose:

Penrose Community Links (PCL) is a CQC registered service specialising in working with the most complex and chaotic clients within social & health care settings. This service will provide floating support to clients within their own home who have physical and/or mental health needs. This includes those who are experiencing psychosis, have a substance misuse or a forensic history, and/or display behaviours that challenge.

The coordinator will be responsible for the recruitment, management and training of PCL staff as well being tasked to respond quickly and professionally to referrals through the deployment of appropriate staff to offer support. The PCL Coordinator will be working alongside senior managers, including the Business and Enterprise Manager, to develop and grow PCL within Camden, Lambeth and surrounding boroughs and will be a key player within the development of key relationships with referring partners.

Aims:  Respond within 24 hours to referrals for care and support  Promote Penrose Community Links throughout the agreed boroughs.  Ensure the highest standards of care and service are delivered across the service  Support Business and Enterprise Manger to drive business forward to achieve/exceed KPI’s and targets  Develop and manage relationships with all parties within the pathway. This may include clients, local authority, customers and their families  Seek opportunities to strengthen professional relationships with business critical individuals and identify business development opportunities and networks  Take accountability for the service and Support Worker compliance ensuring CQC regulations and company policies are maintained.  Implement Penrose complaints procedure and handle complaints effectively against policy.  Be responsible for the recruitment, training and retention of staff

Development

 Work with the Service Manger to establish new relationships with referring partners 1

 Proactively seek referrals and new opportunities for delivering support  Assess referrals as soon as they come in and respond 100% of the time whether that be positively or negatively with regard to the referral.  Report monthly on referrals not taken forward with insight as to what we can do to widen our offer or improve the quality of referrals  Assist the senior managers to plan, develop and implement strategy for development and growth of Penrose Community Links  Attend promotional events to promote Penrose.

Operational:

 Maintain PCL staff rotas. This should consider accurate and appropriate matching of care staff to clients and continuity of PCL workers for the client  Work with referring bodies and establish initial rotas for new care packages.  Ensure clients and their families are up to date with progress and any changes.  Ensure holidays, absences and any changes are accurately recorded on the database and essential cover is maintained.  Update and process all reviews and assessments.  Maintaining accurate records for both staff and clients manually and electronically using ‘Inform’.  Working with Head of Service to ensure compliance with CQC requirements.  Attend training and to develop relevant knowledge and skills.

Management

 Potential for supervision and coaching of support workers, along with regular spot checks  Assisting in office administration  Involved in the recruitment of PCL staff.  Responsible for providing report and submission of information to support Central Office functions such as training record updates, performance statistics for KPI monitoring and Quality & Performance for payroll etc  Ensure all Penrose policies and procedures are appropriately understood, followed and implemented by PCL staff  Work with referring bodies and establish initial support plan for new care packages.

Health & Safety and Environment  Understand and adhere to restrictions and regulations of Service Users where necessary  Liaison with referring agencies and external partners to assess, minimise and manage associated risks  Take responsibility, alongside the Head of Service, for the personal safety of all PCL staff and clients  Ensure all the organisations rules/policies regarding Health, Safety and the Environment are strictly adhered to and that safe working practices are adopted at all times and risk assessments are both carried out and followed

 Responsible for reporting any incidents /accidents and potentially conducting initial investigations into the incident / accident and provide appropriate report  Ensure all RIDDOR incidents are reported in a timely fashion  Responsible for ensuring COSHH and Risk assessments records are maintained and available for reference and use  Assess each individual’s ability and limitations and assign role that are with in their capability.  Assess each site and complete health and safety forms.
