STATE REBATE FINAL ACCOUNTING REPORT NARRATIVE

Complete the Final Accounting Report Narrative offline using this template. Provide the information requested below in order. State each heading as typed below.

When you have completed the Narrative, submit it electronically via the Council’s Grants and Rebates Management System at www.percgms.com. You will be prompted to complete Summary information online. You will upload the Final Accounting Report Narrative (this document), along with Supporting Documents such as deliverables from your project, scanned copies of receipts, etc.

When you complete the online submission by clicking the SUBMIT button, the system will generate a PDF of your final report, which will include: a Summary information page; the document you uploaded as your Final Accounting Report Narrative; and the Supporting Documents you uploaded. This PDF will serve as the completed Final Report submission. You may download and/or print the Final Report PDF for your records. You can access it at anytime via the Grants and Rebates Management System.

1. Detailed timeline of the project, from organization to completion, including project stages and activities.

2. Goals and objectives: Were the goals and objectives outlined in the original funding request achieved as a result of this project? Please explain.

Statement of methods used to evaluate the success of the project.

3. Complete a detailed statement of the cost of the project and final budget accounting. Specifically include; direct costs, salaries, overhead and subcontractor costs, fees and expenses. Page 1 of 2 Statement of how much the state contributed toward the cost of the project in cash, kind and services.

4. Statement of plans to enhance and/or continue project with future rebate funds.
