I. Information for New and Returning Nursing Instructors

 New instructors to Fairview or new to an individual unit must complete 16 hours of orientation on the patient care unit prior to bringing students. See separate document provided to education partners for updated patient care unit manager contact information.

 Electronic Health Record Learnings: Review the EPIC Co-signing tip sheets for the MAR’s and flowsheets/notes (see separate documents provided to education partners). Nursing faculty must co-sign nursing student entries for documentation/flowsheets and on the Medication Administration Record.

 PYXIS: Complete the 15 minute tutorial found on the unit PYXIS medstations. The instructor must be able to guide the students’ individual use of the PYXIS medstation. Only instructors or staff RN’s can remove controlled substances. Per policy, the person who removes the controlled substance must administer the medication.

 Required Learning: Review the Required Learning if you are a new instructor. Required Learning must be completed annually . Documentation forms must be filed at the college or university. The external website is updated automatically every January by the OD and Learning department. An additional HIPAA module has been added for all students. http://www.fairview.org/requiredlearning/

 Policies are available on the Fairview Intranet. The Intranet is available on any patient care unit computer but cannot be accessed off-site by external persons.

 EPIC and PYXIS Codes: Student codes must be requested by completing the Student Request Form. At least three weeks’ notice is needed by IT to assign codes for EPIC. EPIC codes are universal. Additional time may be necessary for Pharmacy to then assign the PYXIS codes. PYXIS access is campus-specific not universal. User ID’s for EPIC and PYXIS are the same 1 Katie Becker, System Academic Affiliation Consultant, Fairview Health Services June 23, 2016 University of Minnesota Medical Center/University of Minnesota Masonic Children’s Hospital – Information for Nursing Instructors, Fall Semester, 2016 (i.e.cbecker1sn) but the initial passwords for EPIC and PYXIS are different. Please note that EPIC will have an individual password and PYXIS will have a password that contains the first and last initial of the student’s name and the last four numbers of the social security number. In the past, students have used the same password for EPIC and PYXIS not understanding that there are two different passwords. To access a computer on the units (network log-in), students will need a standard Fairview ID (i.e.: cbecker1). Again the log-in for EPIC and PYXIS will be a different student ID. This is new.

 Blood Glucose Meter: Those who need BGM codes must complete the competency checklist. If the students do not need glucometer access, let the clinical partner know. Normally, behavioral and Peds units do not grant glucometer codes. A new NOVA glucometer has been implemented system-wide. BGM codes are universal to both campuses.

 Locked doors: Students and instructors will be given access to locked med rooms, stairwells, kitchens, hallways, buildings based on unit requirements and approvals. Specific unit requests will help to determine if various accesses can be given. The request is completed by Fairview in collaboration with the Security Department. Students and instructors must have their proximity reader card badges in the system first before they can be given any accesses by Security. Please allow at least 24 hours for access. Students will not receive any accesses the day that they receive their badges.

 Posting names on units: The names of the nursing students, instructor name, and instructor contact information must be posted on the patient care units. This is in the event there is a question about nursing student documentation and/or medication administration.

 Welcome/welcome back to University of Minnesota Medical Center and University of Minnesota Masonic Children’s Hospital.

II. News

University of Minnesota Health or to be known as M Health. Per public knowledge sometime this fall, Fairview Health Services and University of Minnesota Physicians will join together as one. UMP is a physician group and the physicians are on the faculty of the University of Minnesota Medical School. More to come on this change.

CAPA Pain Scale: In January of 2014, a new CAPA pain scale was implemented on the adult units of UMMC. Educational learnings were sent to all educational partners. The pain scale takes the place of the 1 – 10 pain scale previously used. Please remind students to use the new pain scale when documenting in EPIC. Audits are occurring on the UMMC adult units to assure the new pain scale is universally used and certain audits are indicating that students are still using the 1 – 10 pain scale on adult units. This affects the audit numbers. 2 Katie Becker, System Academic Affiliation Consultant, Fairview Health Services June 23, 2016 University of Minnesota Medical Center/University of Minnesota Masonic Children’s Hospital – Information for Nursing Instructors, Fall Semester, 2016

Patient Care Managers: Please note the separate list of nurse managers. New names will appear on the list. This list will be updated as necessary. Please check if you have questions.

News from IT Provisioning:

On April 4th Application Access and User Security (Provisioning) implemented the LDAP Override function of the Epic EMP user record. Initially this applied to new student nurse requests only starting on the 4th of April; on May 9 we implemented the LDAO Override process for all student records as well as internal users who require more than one account.

Now, all students are assigned a Fairview Standard ID (FVSTD ID) and an Active Directory (AD) account. This allows the student the capability of logging into a local workstation to complete their work when a generic workstation is not available.

The Epic LDAP Override feature allows a student to log in to Epic with their Epic Student ID; but uses their network password to authenticate to Epic. This is especially helpful for students who may hold another position at Fairview. This consolidates the number of passwords they will need to remember but does not remove the need for an Epic Student ID to track their work.

For those students on a campus with single-sign on, they will login to Imprivata with their FVSTD ID, and then into Epic with their Epic Student ID. The password for both logins is the same.

Having a Student ID allows us to create reports with more detail regarding the status of accounts, how many we have and what types of student are in the system.

A list of suffixes for types of students has been created and is always a work in progress as we discover more student types enjoying their clinical experience at Fairview.

In addition, we have created “containers” in the Active Directory (AD) application for students providing them with specific types of access based on the container in which they are placed. Specific AD groups based on the container placement are applied to the student access. Having specific student “containers” provides more efficient and effective access to the student experience. It also, again, allows us more effective reporting capabilities.

LDAP Override also enhances the ability of our applications to provide for the student who eventually takes a position at Fairview. It keeps the FVSTD ID available when employment becomes a reality for a Fairview-trained student. Access as an employee becomes easier to complete without having to “fix” the access that was originally a student. 3 Katie Becker, System Academic Affiliation Consultant, Fairview Health Services June 23, 2016 University of Minnesota Medical Center/University of Minnesota Masonic Children’s Hospital – Information for Nursing Instructors, Fall Semester, 2016

IV. Fairview Photo ID Information

A Photo ID Badge is issued to all Medical Center Employees, Administration, Medical Staff, Residents, Volunteers, Students, Instructors/Faculty, and others providing service at University of Minnesota Medical Center/University of Minnesota Masonic Children’s Hospital for no charge. The badges must not be covered by stickers, tape, or other objects that alter the appearance of the badge holder. Badges are to be worn above the waist facing away from the holder’s body.

IMPORTANT: *****End of Student Clinical Experience*****

Individuals are required to return their Photo ID Badge to the Parking Office on the last day of their clinical assignment. Students and faculty members who will be returning for future clinical experiences may retain their ID badges until the end of the final clinical experience.

Photo ID’s are currently taken in the Parking office on the West Bank Campus (MB218 East) and in the Parking office on the East Bank Campus (Mayo B340). Parking can be arranged at the same time. Students can come between the hours of 8 am – 4 pm. to the Parking offices for photo ID’s. Bring a picture ID. Nursing Students/Instructors will have a hard proximity reader card attached to their photo ID. This reader card is needed to access locked medication rooms on units as well as some locked entry doors or stairwells. Photo ID’s have an expiration date and must be updated if a student returns for several rotations.

If an individual forgets to bring his or her badge or loses it, he or she must report to the Parking Department to receive a new replacement badge. There is a $10 fee. A driver’s license is needed for the replacement badge. If the lost badge is found and brought to the Parking Office then the $10 fee will be refunded. Replacement ID’s can be obtained at either office.

VI. Troubleshooting Contacts

EPIC (Patient Computer):

The individual student or instructor must call the TSC - Technology Service Center at 612-672-6805. The TSC is available 24 hours per day. The student should have the standard Fairview ID available to give to the TSC. Instructors must also have their standard Fairview ID available if having difficulty with their EPIC code access. The TSC should be able to reset the EPIC password. Passwords do expire after 90 days if they have not been used. If you have issues or questions, please contact Katie Becker during business hours.

PYXIS (medstation):

For code issues, call the main West Bank Pharmacy at 612-273-4200 or Wanda Avery at 612-273-4208 or the main East Bank Pharmacy at 612-273-3066 or Kim Arrona at 612-273-7285. Have the student Fairview ID ready. After you enter your initial password (first and last initials and last four of social) for the first time, you will use your fingerprint bio-id from then on as a password. PYXIS is campus-specific not universal. If you have issues, please contact Katie Becker during business hours.

Blood Glucose Meter:

For code issues, ask for the Point of Care supervisor in the main Laboratory at 612-273-3336. BGM codes are universal to both campuses. If you have issues, please contact Katie Becker during business hours.

Locked Areas:

If you as an instructor are having difficulty entering the locked med rooms or other areas with your proximity reader card, have the patient care unit charge nurse call Security at 612-273-4544 for immediate assistance. Security is tightening up on processes to access a variety of units and doors so your students may not have access to certain areas. Access to the locked med rooms will remain the same. Have the last five numbers from the back of your gray proximity reader card ready, full name, and unit. Please note that students must not have an expired photo ID badge. Security has been provided with all student/instructor names. If you can wait until regular business hours, e-mail [email protected] and describe your issue.
