“Shadowing Guidelines” (4/06)

There are four documents attached that are to be used to authorize a person or persons to “shadow” or accompany a physician or other staff primary caregiver during his or her patient visits. These are: Confidentiality Statement (for the person or persons shadowing the BDC physician or staff primary caregiver) Shadowing Script (for the physician) Shadowing Script (for the staff primary caregiver) Shadowing Authorization (for the patient)

Process for authorization: 1. The person or persons intending to “shadow” the physician or other staff primary caregiver during a patient visit must have completed HIPAA training that has been approved by the UCHSC HIPAA Task Force. Training must have been completed through one of the following:  Live training conducted by the UCHSC HIPAA Task Force or online via the UCHSC HIPAA Online Course  The Children’s Hospital approved training  VA Hospital approved training  If training has been completed via another hospital or institution, proof of HIPAA training must be presented to the BDC HIPAA Policy and Security Officer prior to attending any clinic visits 2. The “shadower” must review the Confidentiality Statement and agree to the conditions by signing and dating the form. 3. The BDC physician or staff primary caregiver must read the appropriate Shadowing Script to the patient or patient’s legal representative. The “shadower” must NOT be present when the physician or caregiver is approaching the patient. The shadower may NOT be present until after the patient has signed the authorization (step 4). 4. If the patient or legal representative of the patient agrees to allow the “shadower” to be present during the patient visit, the Shadowing Authorization must be reviewed and signed and dated by the patient. 5. Original Shadowing Authorization form signed and dated by the patient must be put into the patient’s chart. 6. Copies of the Confidentiality Statement signed and dated by the “shadower” must go into the patients’ charts. 7. A copy of both forms must be given to the patient for their personal records.
