KARLA BELL: Welcome to the Disability Benefits 101 Medi-Cal for the Working Disabled Calculator training. If you are working with clients with disabilities who are planning to go to work, one of the biggest concerns is keeping affordable health coverage such as Medi-Cal and In-Home Supportive Services. The good news is that there are several options for keeping Medi-Cal while working and, in this webinar, you will learn more about one of those options, the Medi-Cal Working Disabled Program - a unique Medi-Cal program just for working Californians with disabilities. We will also demonstrate the DB101 Medi-Cal for the Working Disabled Calculator using a case study and you will learn how this tool can be a great teaching tool for both clients and service providers.

I will be your trainer. My name is Karla Bell and I am a Specialist from the California Department of Rehabilitation Social Security Programs Section.

Let's review our objectives for this training. I will start with an overview of the benefits and the basics of the Medi-Cal Working Disabled Program. After learning more about this unique program, I will show you how to locate the DB101 Medi-Cal for the Working Disabled Calculator online. I will provide a demonstration of the Medi-Cal for the Working Disabled Calculator and after viewing this training you will be able to navigate the Calculator screens to the results section and understand the Calculator features and results. Finally, I will review some resources and training tools we have created for you that you can use with your clients or for staff development.

Before we begin learning about the Working Disabled Program, I would like to give a brief overview of Medi-Cal and In-Home Supportive Services or IHSS. Medi-Cal is California's name for Medicaid. The Medi-Cal program pays for medical expenses such as doctor visits, hospital stays, and prescription drugs for people with low income. You can think of Medi-Cal as a single program that you can qualify for in many different ways. The different ways to get Medi-Cal are called eligibility categories. There are over 90 eligibility categories, each with its own rules and requirements. The Medi-Cal Working Disabled Program is just one of the many eligibility categories and is a way to qualify for Medi-Cal if you are disabled and working. The In-Home Supportive Services Program or IHSS provides people who are blind, disabled, or over the age of 65 with personal care and assistance services at home and in the workplace so that they can live independently in their homes. Any California resident living in his or her own home who is already eligible for Medi-Cal can be eligible for IHSS. For more details about either program, I recommend visiting the Medi-Cal and IHSS content sections in the health care coverage section of DB101 at www.DB101.org.

Let's learn more about the Medi-Cal Working Disabled Program. So you might be asking, what are the main benefits of this program for my client? I'm glad you asked, because there are a lot of great benefits for your clients. This program can increase your clients’ self-sufficiency, because it gives them the ability to work and maintain affordable health coverage. This program provides choice to Californians with disabilities who would otherwise be ineligible for no share of cost, full scope Medi-Cal coverage due to earned income and work activity. With this program there is no share of cost, and to access Medi-Cal individuals pay an affordable premium. If you are working with a client that has Medi-Cal with a large share of cost, this program can be an option to eliminate their share of cost if they make work part of their plan. The program has a very liberal definition of employment, there is no minimum earnings criteria, so your client can find the work that fits with their level of disability. They can enroll whether they are working full or part-time, or are self-employed. Many people think of Medi-Cal as a poverty level program, but with the Working Disabled Program, Medi-Cal does not equal poverty. You can earn a lot, build assets and keep Medi-Cal. Medi-Cal has a very restrictive asset limit of $2000, but the Working Disabled Program has several asset exemptions. First, you can save your earnings in a separately identifiable account with no cap or limit. What this means is that your clients can save their earnings for things they need such as medical expenses, or to buy a car, and it will not affect the asset limit. Second, you can have an IRS-approved retirement account such as, an IRA, 401(k) or 403(b) and it will not be counted toward the $2000 asset limit. Your clients probably are not aware that there are some great opportunities for building assets with this program, so sharing this information can be a great incentive for working towards self-sufficiency.

Let's continue with more of the main benefits of this program for your clients. This program is for anyone with a disability; therefore, your clients do not have to currently receive a Social Security disability benefit such as SSDI or SSI in order to qualify. If your client enrolls in the Medi-Cal Working Disabled Program, they won’t lose their In-Home Supportive Services or IHSS if they go to work, which is critical. Also, if your client has In-Home Supportive Services they can transfer some of their IHSS hours into the workplace for assistance with meal preparation and clean-up, and personal care services. Another benefit of enrolling in the Working Disabled Program is that there will be no share of cost for IHSS either. Your client can have multiple types of insurance with the Working Disabled Program, they can use Medi-Cal for wrap-around coverage with employer-sponsored insurance and if needed they can access IHSS for the personal assistance services. I hope this opened your eyes to the many benefits this program can offer your clients. Working and keeping Medi-Cal and IHSS is just the beginning of the benefits this program offers.

Let's review some of the details and unique features of the Medi-Cal Working Disabled Program. Individuals can earn up to $56,892 gross per year and still qualify for Medi-Cal. Yes, you can earn up to $56,000 and still keep your Medi- Cal. Higher gross wages are possible when there are Impairment Related Work Expenses or IRWEs. IRWEs are things you pay for out of pocket, related to your disability and needed for work. Most people don't know about this program, so be sure to let your clients know that with the Working Disabled Program they can earn a lot and they don’t have to be in poverty to qualify or continue their Medi-Cal. To receive Medi-Cal, you pay an affordable premium that is between $20 and $250 a month for an individual, or $30 and $375 a month for a couple. The monthly premium is based on a person's countable income. One of the great features of the Working Disabled Calculator that I will be demonstrating is that it will calculate your estimated countable income and premium.

To qualify for the Working Disabled Program you must be working and earning income. You also have to meet Social Security's definition of disability, but the medical definition ONLY. You are not required to meet Social Security’s income and work requirements. Your countable earnings must be below 250% of the Federal Poverty Level, and I will cover this in more detail during the Calculator demonstration. You must be a legal resident living in California. If your client wants to apply for the Medi-Cal Working Disabled Program, they will need to visit their local county Medi-Cal office to fill out an application. It can be helpful to mention Aid Code 6G when applying if the office is not familiar with the program. The Working Disabled Program has a lot of very unique features. One of the largest benefits of this program is that it was designed with building assets for the future in mind. You can have an IRS-approved retirement account, they are considered exempt and will not count as a resource. If you receive any disability-based income such as Social Security Disability Insurance, State Disability Insurance, Veterans disability benefits or private disability insurance, all of these types of disability-based income are exempt when determining eligibility for the program and the monthly premium amount. While disability income is not counted to determine eligibility for this program, keep in mind that earned income from work may impact these cash benefits, therefore, it is important to check into work incentive programs and work rules available through Social Security and other agencies. There are also several benefits for individuals who have Medicare. If someone who receives Medicare enrolls in the Working Disabled Program, Medi-Cal will pay their Medicare Part B premium which puts about $104 back in a person’s pocket each month. They will also automatically qualify for the Medicare Part D Low Income Subsidy program which helps cover the premium and deductibles of benchmark Part D prescription plans. The prescription co-pays will also be less. Social Security retirement income remains exempt for individuals who were previously on Social Security Disability Insurance or SSDI, which eliminates the age restriction for many people. You can also be temporarily unemployed and remain in the program for up to 26 weeks during your work transition from one job to the next, but you are required to pay your monthly premium during the time of unemployment. Finally, all exempt IRS-approved retirement accounts allowed under the Working Disabled Program will continue to be exempt if you have to transfer to another aged, blind or disabled Medi-Cal program.

Using the DB101 Medi-Cal for the Working Disabled Calculator, your client can predict how their job will affect their Medi-Cal coverage. Service providers can use this calculator during one-on-one counseling to address the client’s fears and questions about benefits and work. Using the Calculator’s simple, plain language results, clients can visualize their options. The Calculator can be used at any time during the employment planning process so be sure to encourage and support the client to start and save a Calculator session for later review. There are tips and links to DB101 content areas and resources all throughout the Calculator designed to empower you and your client with information.

One way to use the DB101 Calculator in client and staff trainings and outreach is by using a case study as an example. I'm going to do a Calculator session using Nelson as an example of how the Calculator can be used to teach about the Working Disabled Program and its benefits. Nelson is a wheelchair user looking for options for affordable personal assistance services. He is 30 years old and he has been paying for his personal assistance services out of his own pocket using an inheritance he received. Personal assistance services are very costly, and he has already spent all of his inheritance. He started working recently as a local newspaper editor and he is earning $4160 a month and has employer-sponsored insurance. He also receives Social Security Disability Insurance, or SSDI and Medicare. Neither Medicare nor employer insurance will provide personal assistance services. He relies on his care providers for assistance with his meals and personal care at home and at work, without them he would not be able to work or live independently. He did not know that he could be working and still qualify for Medi-Cal and IHSS without a share of cost. He tells his case manager his fear that if he cannot find a source of affordable personal assistance services, he will have to quit his job. Lucky for him, his case manager knows about the Working Disabled Program and together they complete a DB101 Calculator session.

This slide walks you through step-by-step directions on how to find the California DB101 homepage and the Medi-Cal for the Working Disabled Calculator. I'm going to open up DB101 and begin our calculator session using Nelson's information.

Okay. So, I have opened up the DB101 landing page at www.DB101.org. On this page we are going to go ahead and click ‘California’. This takes us to the California DB101 home page. Now that we are on the homepage, I will go up to the top to the menu bar and click on ‘Calculators’ tab in the menu and from there, we opened up the calculator’s main page. I'm going to scroll down and find the Medi-Cal for the Working Disabled Calculator and click on the ‘Go’ link. On the Medi-Cal for the Working Disabled Calculator page, click the ‘Start’ button.

Okay, the start page of the calculator provides some basic information. This calculator is designed for California workers with disabilities 18 to 64 years of age. A calculator session will take approximately 5 to 10 minutes to complete. You can also save your session and return to it later. Under ‘What will I need?’, is a description of the types of information you should gather to complete a session such as bank statements, information about your assets and information about your Social Security or state benefits. If you don't know what disability benefits you receive from Social Security you can request a document called the Benefits Planning Query or BPQY from Social Security. To learn more, go to the DB101 homepage and click on ‘What benefits am I on now?’. This article explains how to request a BPQY from Social Security. The BPQY includes details about your Supplemental Security Income or SSI and Social Security Disability Insurance or SSDI benefits. It also has information about your work history, earnings history, health coverage and work incentives you have used or are currently using. Now, let’s return to the start page of the Calculator.

The final question on the start page asks ‘Will my information be kept private?’. All of the information you enter or store in the Calculator is completely private and confidential. It will not be shared with any government agencies or other organizations. Now let's click ‘Start’ to begin the Calculator session.

We are on the first page of the Calculator and this page starts with asking you to enter some basic information. Before starting, let's get familiar with the main features of the pages: At the top there is a two-part Progress bar that tracks your progress as you move through the Calculator. We will start in ‘Current Status’ and once we've entered in all the information needed, we will be able to take a look at the ‘Results’. I want to point out the blue circles with question marks to the right of each question. When you click on a blue circle, information about that question will pop-up to help you answer it. Now, you can also ‘Save’ a session. On the upper right-hand side of the screen you can click on the first blue circle to save your session once you have inputted information. We have already saved the Nelson Case Study, and we will share the username and password for Nelson’s Saved Session later in the webinar. As we go through the Calculator, you will also notice the Grey Tips Sidebar that is located to the right of the questions. There will be different tips on each Calculator page with opportunities to learn more about benefits and work.

So, on the Current Status pages we will answer questions about Nelson's household information, income, resources, expenses and benefits. This page asks questions about ‘Household information’. We’ll start by entering Nelson's birth date, he was born in April 1983 so we will enter 04 1983. The next question asks his citizenship status, and we will select US Citizen. He's a California resident, so on the next question we will confirm that ‘Yes’ is selected. Nelson is not married, so we will leave ‘No’ selected for this question. Finally the Calculator asks what is his disability determination status. Because Nelson receives SSDI benefits he has been determined disabled by Social Security, so we select ‘I have been determined disabled’. This question is asked because one of the eligibility criteria for the Working Disabled Program is that the individual is disabled, but an applicant is not required to be on a Social Security disability benefit to qualify, the County Medi-Cal office can do a disability determination if an applicant is not receiving Social Security benefits. The Grey Tips Sidebar also explains that you can be eligible for this program whether or not you are currently on any type of public benefit program. As we progress through the Calculator, the tips that appear will help us learn more about the program. Now I'm going to click ‘Continue’ at the bottom of the page to advance to the next page.

Several times throughout the session the Calculator gives you a confirmation page where you can check your responses. If you find any incorrect responses click the blue ‘Back’ button at the bottom of the page to jump back to the beginning of the current session to change your responses. If all your responses look correct, you can click ‘Continue’ to go to the next page.

This is the ‘Your Current Income’ page, which asks how much monthly income Nelson receives from different sources. The first question asks, ‘What is your gross monthly income from all your jobs?’. Nelson is earning $4160 gross a month working as an editor at the local newspaper, so I will enter 4160. If you are not sure what gross monthly income means, click the blue question mark. The Calculator explains that gross income is the amount you earn each month from all your jobs before taxes and other deductions are made. The next question asks if Nelson has any self- employment income. He's not self-employed so we will leave it blank. The final question asks ‘What is your other non-disability monthly unearned income?’. If you are not sure what counts as unearned income, once again, click on the blue question mark next to the question and then click on the ‘unearned income’ text in blue with the dashed underline. When we do this, a ‘Glossary’ pop-up will appear. Examples of unearned income are listed such as spousal support, disability insurance or VA benefits. We know the only unearned income Nelson receives is his SSDI benefit, but because this is disability income, it does count for the Working Disabled Program and we can leave the unearned income question blank. The tip on the right has more information about the Working Disabled Program’s income rules. To qualify for the program, Nelson's countable income will have to be below 250% of the federal poverty level. Also, only half of his earned income from work will count and none of the SSDI will count because it is disability income. The great thing about using the calculator is that it will do all of these calculations for you, as you will see later in my results. Let's click ‘Continue’ and move on to another confirmation page.

You might be asking yourself, how much can someone earn and still be eligible for the Working Disabled Program? The grey tip sidebar explains that you really can earn a lot in this program. Someone without any Impairment Related Work Expenses can earn up to $56,000 annually and the program does not count all income. We also learn that the income limits for this program change every year on April 1st. Don't forget you can also use the ‘Back’ button on your web browser at any time to go back one screen if you need to check or change your responses. These responses look good so I will continue.

This is the ‘Resources’ page. It is very important to have the correct information on the total amount of Nelson's resources, such as how much he has in his checking and savings accounts. This program does have a $2000 resource limit, but not all resources count. The Working Disabled Program has some very unique exemptions which are mentioned in the grey tip sidebar on the right. When totaling resources, you can leave out any IRS-approved retirement accounts such as a 401(k) or IRA. The value of your primary house and car are also exempt. Nelson only has $200 in his bank account, so we will enter $200 for his resources and click ‘Continue’.

You're probably getting used to the confirmation screens by now. This page also has a reminder of the $2000 asset limit for Medi-Cal eligibility. Don't forget I mentioned earlier that once you enroll in the Working Disabled Program, you can save your earnings with no limit. So let's click ‘Continue’.

The next page asks about Nelson's Impairment-Related Work Expenses, also called IRWEs. IRWE’s are expenses Nelson is paying out of pocket for items or services needed for work and related to his disability. Adding Nelson’s IRWES are very important because the value of his IRWEs can be used as deductions from Nelson's income, which will help with the countable income calculations for his eligibility and premium amount for the program. If we need help calculating his IRWEs, click on the ‘Help me calculate this’ link on the left side which opens the ‘Estimate Your IRWEs Tool’ that you can use to calculate the total amount of IRWEs. A few categories in the ‘Estimate Your IRWEs Tool’ are: monthly transportation, personal care attendant, medical device costs and prescription drug costs. These are all possible IRWE deductions. Based on Nelson's estimate, we enter $175 for monthly transportation costs for the cost of a driver and we enter $25 for monthly prescription drug costs for Nelson's co-pays. You might have noticed as we were adding Nelson's IRWE costs a running total is automatically calculated at the bottom of the page in the ‘IRWE Total’ box. When you are done, click ‘Use This IRWE’ to return. This closes the ‘Estimate your IRWE’ window and auto fills $200 in the IRWE question box for us. Let's click ‘Continue’.

As we confirm our response for Nelson's IRWE amount of $200 we are getting closer to the results. In the grey tip sidebar, the Calculator provides the Working Disabled Program's current countable income limit of $2328 a month for individuals and a reminder that the gross income limits are actually higher. The current gross monthly income limit for the program is $4741 for individuals. Let's click ‘Continue’ to go to our last pages before the results.

Our final current status page is the health coverage page. This page asks if Nelson is currently covered by no-cost Medi-Cal. We will select ‘No’ because up to now Nelson's resources were too great to qualify for Medi- Cal. He has received an inheritance and has already spent all of it to pay out of pocket for his personal assistance services. He knows that if he is Medi-Cal eligible he will also be eligible for In-Home Supportive Services or IHSS which can provide his personal care at home. He also heard that he can transfer some of his IHSS hours into the workplace so his care provider can assist him with meals and personal care during work. Many people use IHSS for their personal care services and the Medi-Cal Working Disabled Program is a great way to be able to continue their eligibility for IHSS if they are working and earning above the income limits for free Medi-Cal. Let's click ‘Continue’.

This is the final confirmation page, once we click continue the calculator will use the responses we provided to determine if Nelson might qualify for the program and estimate his premium.

All right, here we are at the ‘Results’. We have opened the ‘Results Summary’ page and right away we learn the good news, it appears that Nelson will qualify for the Working Disabled Program. The calculator estimates that his premium will be $200 a month. This quick review of Nelson's results is very general but you can click on links in the menu at the top of the page to get more details. Let's click on the ‘Details’ first, and learn more about how Nelson's eligibility and premium amount was calculated.

You might have noticed the details page has a lot of math. I mentioned earlier that to apply for this program, Nelson will need to visit his local County Medi-Cal office. The details page shows us an estimate of the calculations that will be used by the County to determine Nelson's eligibility based on our responses. To be eligible for the Working Disabled Program, Nelson will need to meet three separate eligibility tests of income and resources and the grey tip sidebar on the right explains these tests. The three tests are countable unearned income, countable earned income and resources. The first calculation on the left is the countable unearned income test. In this calculation we see that there is a $20 general exclusion subtracted from unearned income to calculate countable unearned income. Nelson's only unearned income is SSDI benefits, but remember, disability income is exempt for this program so he has no countable unearned income and the calculator shows Nelson's countable unearned income is zero. Next is the calculation for Nelson's countable earned income. The calculator shows the $4160 earned income that we entered earlier. The $20 general income exclusion is carried over and subtracted since we did not use it above for unearned income. Next, a $65 earned income exclusion is also subtracted. After that, Nelson's $200 IRWE is subtracted. The subtotal is $3875. Next we divide $3875 by two and the result is Nelson's countable earned income of $1937.50. The final step in calculating Nelson's countable income is adding together his countable unearned income, which is zero, and his countable earned income of $1937.50. So, his total countable income is still $1937.50. Next, the Calculator shows you if Nelson has passed all three tests based on the calculations above. First, the countable income test compares his countable income of $1937.50 to the current 250% of the Federal Poverty Level amount of $2327. Nelson's countable income is definitely less than 250% of the Federal Poverty Level, therefore, he passes the countable income test. Second, is the unearned income test. If Nelson had any countable unearned income, it would have to be less than the combined current Supplemental Security Income or SSI and the State Supplement Payment or SSP rate of $866.40 for an individual. The calculator shows Nelson's countable unearned income is zero, so he passes the unearned income test because this is definitely less than the current SSI/SSP amount. And now the final test is the resource test. Nelson only has $200 in resources which is less than the $2,000 resource limit, therefore, he also passes this test. You might be wondering how the calculator determined the $200 premium for the Working Disabled Program. Currently the premium amount is based on Nelson's countable income amount. So based on his countable income of $1937.50, he will pay a monthly premium of $200. Using the details page you can explain to a client how Medi-Cal may determine if Nelson is eligible for the program or not. This is also a great teaching tool for staff that want to learn more about program eligibility. If your client is not comfortable with all the math, you can use this page or just the results summary to communicate the bottom line. Nelson’s bottom line is that the Working Disabled Program will enable him to continue working, get affordable health care and In-Home Supportive Services, and he does not have to quit working to do this. This is a real a-ha moment for Nelson. Let's finish up by reviewing the other details pages starting with clicking on ‘Session Review’ at the top of the page.

This page summarizes all the information we entered for Nelson, if he wants a copy, you can click on ‘Print this Page’ or ‘Email to a friend’ in the yellow box on the right side of the page. Now I’m clicking on ‘Tips’ once again at the top of the page.

That Tips page recaps all the tips we saw in the grey sidebars when we were answering questions. You can also print this page, and it would make a great fact sheet to share with clients or staff. Finally let's click on ‘Next Steps’ at the top of the page.

As a service provider, you can use the Next Steps page as a guide to talk with your client about the steps he can take to apply for the program. This page even has a link to help you find a local Medi-Cal office to go to when he is ready to apply. When applying for Medi-Cal, Nelson will need to come prepared with several documents. He should bring documents that show proof of employment income such as pay stubs or tax returns, information about his resources such as bank statements and proof of his SSDI benefits such as his latest letter from Social Security. The more prepared he is, the quicker the County will be able to process his application and determine eligibility. Nelson also has SSDI benefits and there are many things he needs to consider and work incentives to learn about in the Cash Benefits content section of DB101. He will also need to tell Social Security about his job and report his income. He can use the Benefits and Work Calculator to find out how work will affect all of his benefits. You can also read more about the Working Disabled Program and other Medi-Cal programs in the health care coverage content section of DB101. The interaction between benefits and work can get complicated and there might be times when your client needs to talk to a professional to get help with their questions and benefits. On the right side of every DB101 page is a blue box with the ‘Find an Expert’ link. Clicking on the link will take you to a page that lists contact information for Social Security, Medi-Cal and Medicare assistance and counseling and other advocacy resources.

I'm going to exit the DB101 website and return to our PowerPoint for closing thoughts about the benefits of the Working Disabled Program and the Calculator.

Today we have learned a lot about how to qualify for the Medi-Cal Working Disabled Program and the huge benefits this program offers your clients. This program helps to address the health care barrier so Californians with a disability do not have to choose between taking a job and keeping health care, or let the fear of loss of health care stop them from working. For example, Nelson thought he would either not qualify for Medi-Cal and IHSS or have a large share of cost. Using the Calculator, we discovered that he can access Medi-Cal with a $200 premium and also access the IHSS he needs to remain independent and self- sufficient. Nelson has Medicare and he was excited to learn about having Medi- Cal pay for his $104 Medicare Part B premium and have the Medicare Part D Low Income Subsidy reduce his prescription costs. He also now has some of the best health coverage possible since he has three different types of insurance: Medi-Cal, Medicare and employer-sponsored insurance. With the Working Disabled Program, he will also be able to access additional perks such as saving his earnings in a separate account with no cap and being able to have IRS approved retirement accounts. He is excited to be able to save money for some much needed upgrades to his modified van and for a vacation to Disneyland! The benefits for Nelson will be the same for many of your clients and using the tools on DB101 can empower them with the information and resources needed to take that next step on the journey to employment.

You can review today's Calculator session and results any time on the DB101 website. The username and password listed on this slide for Nelson's saved session is nelson2013. You can show this webinar, use Nelson saved session or create your own case study to use as a Calculator demonstration to train clients or staff on the Medi-Cal Working Disabled Program. Like Nelson, most of your clients are probably receiving multiple benefits, therefore, I recommend trying the DB101 Benefits and Work Calculator because the results will show you how all benefits will change over the five years including options to maintain health care coverage.
