Collection of Award Parchment (Excluding HKCyberU, Outpost, Research, SPEED graduates)
Your award parchment is now available for collection at the Service Centre of the Academic Secretariat at Room M101, Li Ka Shing Tower, The Hong Kong Polytechnic University during the following hours:
Monday to Friday : 9:00 a.m. - 7:00 p.m. Saturday : 9:00 a.m. - 12:00 noon
Special Opening Hours 24 December 2007 : 9:00 a.m. - 1:00 p.m. 31 December 2007 : 9:00 a.m. - 5:00 p.m. 6 February 2008 : 9:00 a.m. - 1:00 p.m.
(you may wish to arrange for the collection during normal office hours to avoid long queues.)
Collection in Person Please bring along: . your Hong Kong Identity Card (or Passport for those without an HKID card) which should tally with the record kept in the PolyU; . the gown return receipt, if you have hired a gown; and . your PolyU Student identity card, which is the property of the University, for cancellation if this has not yet been returned.
Authorization of a Person to collect the Parchment for you The authorized person must produce the following documents: . an authorization letter duly signed by you (a sample authorization form is attached). The letter should state clearly the name and HKID card number (or Passport number for those without an HKID card) of the authorized person; . photocopy of your Hong Kong Identity Card (or Passport for those without an HKID card) which should tally with the record kept in the PolyU; . the gown return receipt, if you have hired a gown; . your PolyU student identity card for cancellation; and . the HKID card (or Passport for those without an HKID card) of the authorized person.
To safeguard your interest, the award parchment will only be given to an authorized person upon the presentation of ALL the above documents. We regret that we will not be able to entertain any requests if any of the above is missing.
You are strongly advised to ensure the safe-keeping of your parchment. The University will under no circumstances issue any duplicate or replacement.
The University is not responsible for the safe custody of the uncollected parchments and it is important for you to arrange for the collection as soon as possible. December 2007
