Section I: Program Performance National Performance Measurement and Program Specific Performance Measurement reporting is done through program Excel chart(s). It is expected that Performance Measurement charts will document by quarter (Q1, Q2, Q3,Q4) and total for grant year. Charts should have Performance Measurement Target or goal listed for each measurement.

For National Performance Measures the chart should track MSY assigned by Performance Measure category (Healthy Futures, Education, etc) as identified in the grant application MSY per performance measure. Measurement categories for National Performance Measures wording on the performance measurement chart should match the phrasing used for measurement in the performance measurement section of your grant application.

Section III: AmeriCorps Member Enrollment This narrative paragraph discusses member enrollment. What is the program’s enrollment rate? If the enrollment rate is less than 100%, describe the steps you will take to improve. Additionally, please list the number of unfilled refill slots and your plan to fill them by year end. Enrollment rate is calculated by dividing the regular slots filled plus the refill slots filled by the regular slots awarded (regular slots + Refill slots)/regular slots awarded. Discuss compliance towards 30 day enrollment.

Section IV: AmeriCorps Member Retention This narrative paragraph discusses member retention. What is the program’s retention rate? If the retention rate is less than 100% describe the steps you will take to improve. It is sometimes helpful to include information on the number of members exited for Personal and Compelling circumstances. Retention rate is calculated by dividing the number of members exited with award (full or partial) by the number of members enrolled. Discuss compliance towards 30 day exit. Section V: Inclusion

Section X: Significant Program Changes Please document any significant changes to the AmeriCorps Program, Grants management staff, partners, host sites or site supervisors and if those changes will impact members or program and your plan to address. Explain any significant program changes that might impact performance measures.
