Guidelines: Present one of your actual patients that you cared for in the past week of clinical:

 Give a two-minute focused hand-off report on your patient.

 What patient problems/issues justify your selection of this patient’s highest priority

nursing care need?

 How did your care make a difference in the quality of this patient’s hospital stay on your

shift?

 How did you feel about the care you and the healthcare team provided to this patient?

 If you cared for this patient tomorrow, justify changes you would make to the patient’s

plan of care.

 What was your patient’s teaching and/or discharge needs? How did you meet them?

 Explain how you met the QSEN competencies. Did you encounter any difficulties in

meeting them? Why? How did you overcome these difficulties?

Peers’ Responsibilities during handoff report:

 Write down each student’s handoff report

 Consider whether the report:

 Was organized and focused?  Contained the most essential information you needed?

 Omitted any key data? Give examples
