 Groups are only eligible for funding from the Student Foundation once every 2 academic years

 Social Fraternities or Sororities, as well as faith-based organizations are not eligible

 The Student Foundation will not provide funds for membership dues, gasoline, or food for group members to consume (events partaking in the distribution of food to non-members shall be considered)

 Please complete the first page of this application by hand, and type your responses on a separate, attached document

 Please limit your presentation to 10 minutes, as there may be other groups waiting to present

 Any other questions regarding the funding request process can be directed to the Funding Request Director (contact information is provided on the first page of this form)

Pre-Funding Agreement

 As representative of ______student organization, I (please print name)

______agree to the following stipulations for receiving funds from the OSU Student

Foundation: (1.) Documented proof of expenditure utilization will be provided to the Student Foundation in the form

of cash receipts upon receiving funds either electronically to the Funding Request Director, or via printed copy to

Student Events and Activities (MU103) immediately upon completion of this event/activity, or for start-up

organizations, upon finalizing all budgetary expenditures for the academic year. (2.) If proof of foreseen expenditures

in the form of cash-receipts cannot be provided, we will agree to provide the Student Foundation with a brief follow-

up presentation explaining how the funds were used and what was learned from the experience.

 Failure to provide either financial documentation or a follow up presentation within TWO WEEKS of the funded

event will lead to a forfeiture of funding eligibility during the next available academic year. Eligibility will begin

again 3 years after the funds were disbursed.

 All non-utilized Student Foundation funds shall be returned within the same two-week period.
