INDICATE ANY COMPETITIVE COMPANIES. We will try to recognize this in exhibit placement, but cannot make any guarantees.

IF SHIPPING PACKAGES TO THE HOTEL, PLEASE INDICATE TO WHOM THEY WILL BE ADDRESSED. The holding and receiving fees will be charged to the following individual’s hotel room and must be paid upon checkout.

EXHIBIT ASSIGNMENTS & HOURS Exhibit space will be assigned at the sole discretion of Florida Housing Coalition based on date of request and payment. Regarding the Housing Expo, preference will be given to Partners for Better Housing at the Sponsor and above levels. Expo displays may be set up Sunday, September 8, 2012, between 3 p.m. and 7 p.m. The Hotel requires that hotel staff must hang all banners. Hotel staff will be available only during these hours prior to the opening of the conference. The Expo Hall will be locked September 8 at 7 p.m. and September 10 at 5:30 p.m. Break down may begin on Tuesday, September 10 only after 4 p.m., and must be completed by 6 p.m. The Expo Hall will also be locked during lunch during at which time the Expo Hall must be cleared.

Exhibitor Name Badges are issued to exhibitors who are not registered for the conference. If individuals not registered for the conference would like access to food functions, please indicate the food functions you would like to attend as seen below. Payment must be received in advance of the conference.

Forms may be faxed to the Coalition at 850.942.6312; however, the exhibitor reservation request will not be accepted or assigned until all forms and payment are received. Changes for the name badge(s) can be made until 5 p.m. EDT, August 2, 2013, at no charge; after this date, name badge changes will be accepted at a charge of $50 each. Please print neatly/type and complete this form for each badge as you wish it to read.
