Student Survey – Given to 8th and 11th Graders
Students will answer the following questions using the Likert Scale of: Strongly Agree, Agree, Disagree, and Strongly Disagree.
Fine Arts
1) My school encourages participation in the Fine Arts Programs, such as Art, Theater, Music, Band competitions, etc.
2) My school provides me with a variety of course offerings in the fine arts.
Wellness and Physical Education
1) I am offered a variety of school options for physical education and wellness, such as health classes, PE classes, athletics and clubs.
2) The school provides the equipment that I need to be successful in a physical education, athletics and/or health programs.
3) The school provides counseling services and/or programs to help with my social and emotional needs.
Community and Parent Involvement
1) My school encourages my parents to be a part of my education at this school.
2) My school provides convenient and effective ways for me and my parents to communicate with my teachers and administrators.
3) My school welcomes my parents to visit the school.
21st Century Workforce Development Program
1) My school provides met with Career and Technology Education (CTE) Courses and Pathways to meet my needs and career goals. (CTE Courses include but at not limited to, Animal Science, Floral Design, Information Technologies, Digital Interactive Media, Video Game Design, Human Resource Management, Business Law, Culinary Arts, Business Information Systems, Principals of Agriculture and Natural Resources, Agricultural, Mechanics and Metal Technologies, Restaurant Management, Manufacturing Technology Academy, Information Technology and Security Academy, and Computer Programming.)
2) My school provides me with counseling services to help me understand career options and pathways to meet my future college and career goals.
3) My school provides me with the support I need for academic and career planning. 4) I believe I am learning what I need to learn at this school to be successful in the next grade level and after graduation.
Second Language Acquisition Program
1) My school provides multi-cultural activities that celebrate various cultures and people.
2) My school offers me language learning opportunities that meet my needs and future goals.
Digital Learning Environment
1) My teachers use technology in the classroom to assist in my learning.
2) I am provided with the technology needed to be successful in school and after graduation.
3) Digital learning in the classrooms helps me be a more efficient learner.
4) The school provides me with appropriate technology to support my learning needs.
Drop Out Prevention Strategies
1) My school offers a variety of extra-curricular activities, class options, and clubs that interest me.
2) My teachers and administrators recognize my academic, athletic and/or classroom achievements.
3) The school provides me with alternative ways to obtain credits toward graduation. (i.e., credit recovery, shortened schedule, online learning, etc.)
Educational Programs for GT Students
1) My school offers AP and Dual Credit Courses that meet my needs for college and career advancement.
2) The GT program at my school helps me achieve my educational goals.
