Association of Fundraising Professionals—Greater Tri-State Chapter National Philanthropy Day® 2016 Nomination Deadline – Monday, June 13th, 2016 National Philanthropy Day® Luncheon – Thursday, November 17, 2016

NOMINATION GUIDELINES: 1. Nominations must be postmarked on or prior to Monday, June 13th, 2016. 2. The nominee may be involved in the preparation of this form and must be willing to accept an award if chosen (we could accept a resume or curriculum vitae, if available). 3. Only nominations with all three sections completed will be considered (see below). 4. The judging committee will base decisions solely on submitted information. 5. Mail nomination form and required information to: AFP National Philanthropy Day®; c/o Alissa Riegler, Clarke University, 1550 Clarke Dr., Dubuque, Iowa 52001.

NOMINATION INFORMATION: (Important: All three components listed are required for nomination.)

1. NOMINATION FOR: (please select one; photocopy form for additional nominees)  Outstanding Individual Philanthropist: This award is presented to an individual or family with a proven record of exceptional generosity given their capacity to give; who through direct financial support has demonstrated outstanding civic and charitable responsibility; and whose generosity encourages others to take leadership roles in philanthropy and community affairs.  Outstanding Volunteer Fundraiser: This award is presented to an individual who has demonstrated exceptional skills in coordinating and motivating groups of volunteers for one or more major fundraising project(s) for the benefit of a charitable institution, as well as a commitment to the advancement of philanthropy.  Outstanding Philanthropic Organization: This award is presented to a business, corporation or foundation that has demonstrated outstanding commitment through financial support and encouragement and motivation of others to take leadership roles in philanthropy and community affairs.  Other Awards: The Greater Tri-State Association of Fundraising Professionals may, on occasion, present other awards to persons or organizations deserving of special recognition.
