APPENDIX

Page 1: Responsible Conduct of Research (RCR) Survey Thank you for participating in this important study regarding standards of conduct in the practice of research. Your responses are important. This survey is designed to inform policy and practice regarding the delivery of education in the responsible conduct of research. The survey has five sections: Authorship, Collaboration, Data Management, Teaching & Learning, and Demographics. Based on a pilot study, the survey is expected to take about 15 minutes to complete.

Page 2: AUTHORSHIP Please respond to the following questions about standards of research conduct for authorship. Standards: Accepted practices or approaches to the responsible conduct of research; standards might be written (e.g., regulations or guidelines) or unwritten (e.g., customs or practices). Authorship: Allocation of credit to the creators or originators of scholarly works - books, journal articles, research monographs, etc. For those questions which begin "In my experience", we are asking you to reflect on your experiences as a researcher in the field with which you are currently affiliated/engaged. You should interpret "experience" broadly: it will include what you know of your own practices as well as those of others. In my experience: Q1.
