2016 Gift of Sight The Cost of Courage Foundation HONORING OUR HEROES

Furlong Vision Correction Gift of Sight Program for Veterans Furlong Vision Correction’s (FVC) Gift of Sight (GOS) program will provide LASIK to qualified veterans who can realize significant benefits from vision correction surgery.

THE GIFT OF SIGHT PROGRAM Dr. Furlong will donate laser vision correction surgery (LASIK or PRK) or Keratoconus procedures and the associated medical visits before and after surgery. This includes a comprehensive eye examination to determine if a prospect is a qualified candidate for the procedure and as many follow up examinations as needed (typically 3-4). In the event the patient needs an enhancement (typically 3-6 months after the initial procedure), the procedure would be performed at no cost to the patient.

PATIENT ELIGIBILITY CRITERIA - Candidates are American veterans who were on active duty, honorably discharged and meet one or more of the following criteria: o Financially challenged o Physically or mentally challenged o Occupationally or developmentally challenged - Consensus that the procedure will benefit their life/lifestyle or occupational fitness - Have a stable prescription. - Be able to see well with their glasses or contact lenses - Have healthy eyes and meet other medical testing. - Be able to follow directions and be comfortable with undergoing a medical procedure

PROCESS A timeline will include: - Patient identification including vision history - A brief history of the client’s work and living background - Ability to get to post operative appointments including the day

TIME LINE - Candidate submission form deadline: Friday, July 1, 5PM - Consultation date: Tuesday, August 9 - Candidates will be confirmed and notified within 2 days following consultations - Surgery date: Thursday, September 8

TESTIMONIALS All GOS patients will agree to participate in our testimonial program. This program can include photos and video of the procedure, video or written testimonial by the patient and publicity and marketing initiatives.
