January, 2011 to Present Gundersen Fertility Center, Onalaska, Wisconsin Clinical Manager of Fertility Responsible for department staffing, budgeting, inventory, and overall program quality. Integrate with other departments for the purpose of service outreach: Cancer center, urology, endocrinology, bariatrics, and regional services. Involved in marketing strategies such as advertising, promoting services locally and nationally, website upgrades, outreach communications. Responsible for impact planning to achieve goals in employee engagement and patient satisfaction.

September, 2007 to Present Gundersen Fertility Center, Onalaska, Wisconsin Embryology Laboratory Director (ABB) and Technical Supervisor (ABB) of Embryology and Andrology Laboratories Provide all patient consultative services related to IVF. Set up state of the art IVF laboratory, including ordering equipment and supplies, setting up equipment, designing protocols and forms, re- designing the gas delivery, backup, filtration, and alarm systems, re-training of laboratory staff, developing QC, QI, and QA programs, bringing the lab into compliance with CAP (for RLAP certification), CLIA, and FDA regulations. Direct and supervise two technicians in Andrology. Design and maintain inventory control and quality testing of supplies. Responsible for consultation with physicians regarding laboratory testing and interpretation of patient results. Designed and implemented communication systems between the laboratory, the physicians, the clinical staff, and the patients regarding IVF and andrology laboratory results. Aided in design and implementation of patient consents. Set up and maintain a cryobanking system for eggs, sperm, and embryos, including FDA compliance, inventory control and validation, shipping tissue if necessary, and storage billing. Design database for IVF cycles, establish operating parameters, and perform statistical analyses of patient results for quality control and quality assurance. Responsible for annual CDC reporting and validation of results. Senior embryologist for IVF procedures, including transvaginal aspirations, insemination, denuding of oocytes, ICSI, fertilization checks, cleavage and blastocyst checks, embryo selection and grading, embryo transfer, day 3 embryo biopsy, trophectoderm biopsy, laser assisted hatching, vitrification and warming of embryos, and oocytes. Completed egg bank training with My Egg Bank, Northwest Cryobank, and World Egg Bank. Senior andrologist responsible for sperm preparations for IUI and IVF, semen analyses, morphology, cryopreservation, PESA and TESA preparations, and retrograde semen analyses. Coordinated partnership efforts with Oncology and Urology departments to provide fertility preservation services for patients at risk of losing fertility. Spearheaded IRB acceptance for oocyte and ovarian tissue cryopreservation. Provide consultative services for cancer patients interested in pursuing fertility preservation services. Perform and maintain quality control analyses on all consumable supplies, relevant equipment, and media. July, 1997 to July, 2007 Advanced Fertility Center of Chicago, Gurnee, Illinois ABB Technical Supervisor of Embryology, Andrology, and Endocrinology Laboratories. Senior embryologist – performed all procedures on approximately 2000 IVF cases, including transvaginal aspirations, inseminations, denuding of oocytes, ICSI, fertilization determinations, cleavage and blastocyst checks, embryo selection and grading, embryo transfer, AZH, cryopreservation and thawing of embryos at the 2PN, cleavage, and blastocyst stages. Designed and implemented culturing systems. Senior andrologist – responsible for sperm preparations for IUI and IVF, semen analyses, morphology, cryopreservaton, PESA and TESA preparations, fructose test, retrograde semen analyses, and microsort preparations. Supervised support technicians. Interviewed and trained all technicians that performed embryology, andrology procedures and/or ran bloods for endocrinology. Responsible for ALL aspects of laboratory compliance and certification for CLIA and CAP. Input data for SART and NASS and complete data validation for the annual CDC reports. Facilitated design of new laboratory in 2000 – including air filtration, gas delivery, emergency backup, and off site alarm systems. Completed ALL protocols, procedures, and employee documentation of training and risk for OSHA compliance. Wrote and revised as necessary all laboratory policies, procedures, worksheets, and reporting sheets. Completed all requirements with protocol changes in the laboratory to meet and maintain compliance with FDA regulations and good tissue culture practices. Responsible for all aspects of inventory control for the laboratory – including ordering and testing of consumable supplies, ordering equipment, price negotiations, and equipment maintenance agreements. Completed all on-going maintenance and calibration of equipment. Designed and performed quality control procedures for testing of all supplies and equipment for use in sperm preparation and IVF with the use of sperm survival assays and mouse embryo assays. Maintained constant media quality control by pH and osmolality analysis. Designed and maintained an inventory control system for embryos and sperm stored in the laboratory, including quarterly tank inventory, documentation, and storage billing. Implemented a system of communication with all levels of the laboratory, the physicians, the clinical staff, and the patients regarding lab results and IVF cycle information.

July, 1995 – July, 1997 Center For Human Reproduction, Chicago, Illinois – merger with Reproductive Biology Resources Embryologist and Andrologist Responsibilities included all aspects of embryology, including oocyte retrievals and evaluation, sperm preparations, sperm and embryo cryopreservation and embryo transfers (IVF, ZIFT, and TET). Micromanipulation experience included preparation of gametes, set up, ICSI, and AZH. Additional laboratory duties included quality control, media preparation, and performance of mouse embryo assays.

June, 1994 – July, 1995 Reproductive Biology Resources, Glenview, Illinois Embryologist and Andrologist Performed oocyte retrievals, evaluations, culture, inseminations, cryopreservation, GIFT and ZIFT. Prepared semen samples for IVF and IUI. Responsible for semen analyses, anti-sperm antibody tests, fructose tests, and isolated serum for use in IVF. Performed all aspects of mouse embryo assays for quality control, including stimulation, evisceration of ovaries, recovery of zygotes, culture and assessment of embryos. January, 1991 – January, 1994 Medical College of Wisconsin, Milwaukee, Wisconsin Managed Primary Tissue Culture Core Designed and conducted research experiments on the limbs of chick and quail embryos on the third and fourth day of embryonic development, including in vitro culture of the ectoderm, microdissection, immunocytochemistry, histochemistry, and in situ hybridization. Maintained inventory of the laboratory.
