Do not write anything in the shaded areas designated “For Office Use Only”. 4. Make a Pay Order / Bank Draft for Tk. 2500.00 payable to “DBA, IBA”. 5. Attach your research proposal (within 1,500 words) with this application. 6. Submit the filled-in form & the Pay Order /Bank Draft to the DBA Program Office between August 16 to September 10, 2014 (Time: 9:00 am to 6:00 pm except public holidays). 7. Add additional paper, if necessary, to complete a section. 8. After submitting this form, collect your Admit Card from DBA Program Office on the assigned delivery date.
