Context The post is to support the work of the Crisis Intervention Community Support Service (CICSS) and to work alongside our long established Home from Hospital service at QMC Campus at Nottingham Hospitals Trust. The service is focused within Clinical Commissioning Group in Nottingham North East and West areas. The British Red Cross is the largest voluntary humanitarian organisation in the world. It promotes humanitarian values and provides emergency help to people in short term crisis in the UK and overseas. Within the UK, the British Red Cross operates through 33,000 volunteers and 2,800 staff. In Derbyshire & Nottingham our Home from Hospital, Care at Home and Mental Health Services offer support to the frail and vulnerable to enable them to maintain independence in their own home

Overall Purpose of the Post To assist the service co-ordinator / assistant coordinators by undertaking duties required too meet the service users’ needs in relation to worsening health needs. In order to reable service users to remain in their own home during their health related crisis period. The post is based within Crisis Intervention Community Support Service (CICSS) Team providing reactive, focussed and flexible support to vulnerable adults at a time of worsening health crisis. The post will report directly to the shift supervisor - Crisis Support Service Co-ordinator or Assistant Coordinator. The post will be responding to referrals from Clinicians and all Community Health Professionals in Nottingham North East and West area. Initially you will be required to have the ability to assess service users’ needs and complete a Care Plan for the individual. The ability to offer personal care, medication prompting, practical, emotional support and signposting in the service users own home. Having a focussed and calming manner and the ability and knowledge to read the situation and report back any changes in users’ condition. To have a flexible and reactive approach in a pressured environment and the ability to work as part of a team within the

Main Duties and Responsibilities > To undertake shifts on a rota basis over a seven days per week offering crisis support in the community within the Nottingham Clinical Commissioning Groups to prevent individuals being admitted into hospital

> To have the ability to provide assessment of service users and to develop a care plan of needs

> To assist individuals with personal care, medication prompts, light meal preparation/drinks, shopping, practical/emotional support. Assisting in a calm and timely manner with anything within the needs of someone experiencing a worsening health crisis within their own home

> To ensure a safe as possible living environment for individuals whilst respecting the service user choice and rights

> To do follow-up calls as required and direct and assist service users with activities to reable their daily living skills within their capability

> To maintain appropriate records in the care plan and update office records at the direction of the service co-ordinator /assistant coordinator

> To report any changes to service users health needs immediately to shift Line Manager and to be aware of escalation procedures of the service

> To have the ability and knowledge to offer signposting in support of service users needs

> To assist the co-ordinator in general duties to ensure the smooth running of the service

> To ensure working within Care Quality Commission(CQC) Standards

> To ensure service users are not put at risk as a result of home care activities

> To maintain confidentiality within British Red Cross Policy and Procedures

> To uphold the Fundamental Principles of the British Red Cross at all times

> To work within the British Red Cross Equal Opportunities Policy

> To undertake any other duties as requested by the Line Managers

Education/ > Educated to GCSE level or equivalent by experience Training

Experience > **Participation in a multi-disciplinary team environment > **Dealing with the public in person > **NVQ Level 2 Health &Social Care & experience of working in the care/voluntary sector > Specialist training as required within Care Sector

Knowledge/ > **Good geographical knowledge of the service area Technical Skills > **Knowledge of handling clients who may be disabled and/or frail, cognitive impaired, dementia and mental health issues > **Ability to deliver personal care needs with dignity and respect

Competencies > **Excellent interpersonal and communication skills > **Ability to work as part of a team and also on own initiative > **Ability to work under pressure in a timely manner

Behaviours > Ability to work at locations other than normal work base within the Community > **Ability to empathise with clients, many of whom will be vulnerable and elderly.

Essential > **Driving Licence > **Car Owner > **Able to work on shifts pattern over 7 days per week 365 days per year > Able to travel within the Nottingham area as appropriate to the needs of the job > **To work flexibly to meet the needs of the service
