PROGRAM SPECIALIST

The Fund for Public Health in New York City, (FPHNYC) is a 501(c)3 non-profit organization that is dedicated to the advancement of the health and well-being of all New Yorkers. To this end, in partnership with the New York City Department of Health and Mental Hygiene (DOHMH), FPHNYC incubates innovative public health initiatives implemented by DOHMH to advance community health throughout the city. It facilitates partnerships, often new and unconventional, between government and the private sector to develop, test, and launch new initiatives. These collaborations speed the execution of demonstration projects, effect expansion of successful pilot programs, and support rapid implementation to meet the public health needs of individuals, families, and communities across New York City.

PROGRAM OVERVIEW The NYC Department of Health and Mental Hygiene (DOHMH), Primary Care Information Project (PCIP) is devoted to improving population health in underserved neighborhoods through health information technology. As part of PCIP, the Cancer Prevention Program’s (‘CPP’) mission is to reduce cancer’s incidence and mortality, and to promote the overall health of at-risk New Yorkers. The reduction and elimination of health disparities is a core component of all CPP initiatives. Colorectal cancer (CRC) prevention is a critical CPP focus. CRC remains the second leading cause of cancer death in NYC, and yet is one of the most preventable cancers. Strong evidence supports the effectiveness of CRC screening, and a decade ago NYC launched an aggressive campaign to increase screening rates. In the past decade, NYC’s CRC screening rate increased by over 60% and racial and ethnic screening disparities have been eliminated between black, white, Asian and Hispanic New Yorkers. However, disparities persist and the screening rate increases have levelled off. Insurance status remains a significant inequality for CRC screening, with the rate for the uninsured significantly lower than those with insurance.

The CPP is piloting a new program that addresses this inequality for the uninsured and also develops primary care based quality improvement strategies to improve screening rates. The program provides free colonoscopy screening for uninsured New Yorkers, by linking large scale primary care practices serving underserved communities, such as Federally Qualified Health Centers (FQHCs) and Community Health Centers (CHCs), with free standing Endoscopy Centers (ECs). The CHC refer their uninsured patients to the endoscopy centers that provide free colonoscopy screening. The project leverages a charity care requirement for ambulatory surgical centers

POSITION OVERVIEW PCIP is seeking a Program Specialist. Reporting to the Program Manager, the Program Specialist will be responsible for assisting in the recruitment and outreach to CHCs and ECs to solicit participation in NYC Community Cares Project (CCP). In addition to serving as a liaison to participating sites within the CCP, the Program Specialist will coordinate monthly data collection and feedback; ensuring project outcomes are monitored and evaluated as well as participate in the implementation and development of CHC-based initiatives such as patient navigation and care coordination. The Program Specialist responsibilities include the following:

RESPONSIBILITIES  Assist NYC Community Cares Project (CCP) team in recruitment and outreach to primary care centers and ECs to solicit participation in project, which includes introductory meetings, phone calls and/or emails to convene participating sites and establish workflows for direct referrals for colonoscopy  Coordinate CCP meetings and phone calls with participating centers, specifically assisting with scheduling and logistics of meetings, and managing minutes and next steps  Manage a case load of sites participating in CCP and serve as a liaison between primary care and endoscopy center staff; which includes ongoing check-ins and troubleshooting  Provide on-site technical assistance to CCP partner sites to encourage quality improvement work through implementation of best practices to increase colorectal cancer screening rates

 Coordinate/track shipments and use of bowel prep donations, as well as seek new donations for the Project  Implement, track and manage patient gift card incentives at primary care centers  Collect data from participating sites on a monthly basis and generate quarterly benchmarking reports for a caseload of participating sites  Assist in survey assessments and analysis for a case load of participating sites  Assist with convening all CCP partners for periodic learning collaborative-style meetings

 Assist Cancer Team with administrative tasks, research activities and additional projects as needed

QUALIFICATIONS  Bachelor’s Degree in health-related field and a minimum of 3 years of experience in a related field; public health experience a plus; OR  Master’s Degree in Public Health and a minimum of 1 year of experience in a related field; public health experience a plus  Intermediate Excel skills for data reporting, analysis and feedback  Keen attention to detail and ability to prioritize and handle multiple assignments  Ability to work independently and exercise a high degree of initiative to accomplish tasks and solve problems  Excellent interpersonal, collaborative and team skills are a must  Strong written and oral communication skills  Willing to travel locally  Bilingual in English/Spanish a plus
