 SAMPLE SIZES are available to try on at the spirit store during store hours or at Monogram Specialties (please call to set up a time to stop over) 847-566-7676.

 Fill out form completely. (Name & Phone #s are important)

 ** Remember #s count as 2 patches ($18 not $9)

 If TALL size is needed make sure you put “T” by your size. Example: XL – T

 Place order form, patches, numerals, letters etc. in a Ziploc bag with your payment.

 Put you name on the Ziploc bag and drop-off at Monogram Specialties or the MHS Spirit Store.

NOTE: Not all sizes are available immediately. If a size needs to be ordered please expect a turnaround time for your jacket of 5 to 6 weeks. If your size is available please expect a turnaround time of 2 to 3 weeks for completion.
