Services Shall Include but Not Necessarily Be Limited To

Social Service Consultant

It is agreed between Judy Goldberg, M.A./JM Enterprises, LLC (Social Service Consultant) and that this individual will be considered an Independent Contractor/Consultant with respect to all services performed for It is clear between both parties that will control the general nature of the services to be performed, but Judy Goldberg has the right to control the specific procedure for accomplishing these services.

The services shall begin on and continue for a period of one year at a rate of $100.00/hour inclusive of all expenses. or Judy Goldberg (JM Enterprises, LLC) may terminate this agreement upon written notification at any time.

Services shall include but not necessarily be limited to:

 annual review of written policies, plans and procedures;

 annual review of (in-service) educational programs;

 availability by telecommunication for advice regarding problems;

 availability, in person, of the consultant to the program;

 consulting with administration and staff about specific problems;

 acting as a resource person to staff and parents;

 documenting the activities and observations required in a consultant log that is kept on file at the facility for not less than two years.

A retainer, to be used toward services, of $100.00 will be paid to commence this contract. Billing will be done by Judy Goldberg and will include: the type of service, date of service, hours of service and expected payment. Payment is expected within 15 days of the date of invoice In signing this agreement, all parties declare that they fully understand the terms of this agreement
