Amazing Creations Pet Salon LLC (ACPS) is committed to providing a low stress grooming experience for your pet. Does your pet have any medical conditions or allergies? Does your pet have any other special concerns that we should be aware of? If yes, please explain.

Health or Medical Problems and Senior Pets: Grooming services can sometimes be stressful, especially for a senior pet or a pet with health problems. Grooming can expose hidden medical problems or aggravate a current medical problem during or after the groom. In the best interest of your pet, this release form will give ACPS permission to obtain immediate Veterinary treatment for your pet should it be deemed necessary. It is agreed that all expenses for Veterinary care will be covered by the pets owner upon signing this release form. ACPS is not responsible for allergic reactions of any sort.

Vaccination/Veterinarian Information: By signing this release, you verify that your pet is current on its vaccinations. Proof of vaccination or titer testing shall be provided upon request from ACPS. Also know and understand that even a fully vaccinated pet can still contract illness and you will hold harmless ACPS if your pet becomes ill. We maintain a clean and sanitary environment and will not allow any known sick pets into our establishment.

Veterinarian______

Aggressive or Dangerous Pets: I agree to inform ACPS prior to grooming if my pet has ever bitten any person or dog, or has any aggressive tendencies whatsoever. Muzzles may be used if necessary. I understand that I will be held liable for any bites that require medical treatment and or any property damage caused by my pet. ACPS reserves the right to refuse service for any reason.

Ear Hair: I understand that there are health benefits and risk to plucking ear hair. ACPS will not pluck infected ears. In some circumstances ACPS will refer you to your vet to pluck ear hair. I wish my pet’s ear hair to be plucked Yes No Anal Glands: I understand that ACPS can gently express anal glands externally when requested. Glands that are infected, impacted, or for other various reasons may not empty from external expression. Some pets may need their glands expressed internally at a veterinarian facility. ACPS cannot determine your pet’s needs in this matter and will not be responsible for any consequences of anal gland expression. I would like my pet’s glands to be expressed Yes No

Appointments: A missed appointment is a loss to everyone. Please give ACPS 24 hours notice to avoid a cancelation fee of $25 and allow this time to be offered to someone else. Please arrive within 15 minutes of your scheduled appointment time. Failure to do so could result in the loss of your appointment and a cancellation fee.______

I understand that ACPS policy is to call me when my pet is done. Stopping by before receiving a call is detrimental to the safety and efficiency of the grooming service.______

ACPS may use photos of my pets on social media or in advertising. Yes No

Hold Harmless Agreement: By signing this release I/we agree to hold ACPS harmless from any illness, injury, damage, loss or claim arising from any condition of my pets, either known or unknown to ACPS. It is also further understood and agreed that terms of this agreement can change at any time, without notice and will overwrite any and all prior signed contracts and releases. It is further understood that this release applies to any and all pets groomed.
