We are proud of your child's skills and want to show them off! As working parents we know how hard it is to watch all your child's events and know what they are up to. Maui Tumblers will periodically take photo/video's of your child's skills and share them with you. We may also use them as promotional items via internet, printed copy, educational material etc. By signing this form, I grant Maui Tumblers permission to the rights of these images, likeness and sound of my child's voice as recorded on audio or video tape without payment or any other consideration. I understand that these images may be edited, copied, exhibited, published or distributed and waive the right to royalties or other compensation arising or related to the use of these images or recordings. I will be consulted about the use of the photo/video's for any other purpose than those listed above. There is no time limit on the validity of this release nor is there any geographic limitation on where these materials may be distributed.

By signing this form I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby.
