(There are 4 pages to this form). Due to the quantity of callmakers that request to be featured in these calendars, submitting this form does not necessarily assure that you will be featured in the next issued year, however, you will be featured in a future issue unless for some un-seen reason I must stop publishing. My goal is to feature 12 different callmakers every year. As soon as the 12 callmakers are “locked” down for a particular year, I will announce them. I do not show favoritism on who is selected for the next issue, but callmakers who may be elderly or have health related problems will have priority. Presently, there are no costs to callmakers and I plan on keeping it that way. I am big on communication throughout the process of getting these finalized and hope that you will be as well.
