Harbor Regional Center is seeking proposals for the development of specialized resources for clients moving from the state developmental centers.

If you are interested in developing these services please complete a proposal as described below. Applications that are submitted with incomplete information or proposals that do not meet the basic requirements will be eliminated. This RFP does not commit HRC to procure or contract for services or supports.

Harbor Regional Center may elect to fund all, part, or none of the project, depending on funding availability as approved by the Department of Developmental Services and the quality of the proposals received. Please refer to the attached project description and requirement, which briefly describe the service concept and possible resource development funding availability.

Summary Project:

Harbor Regional Center is seeking development of day activity sites that each provides specialized medical and/or behavioral services for individuals who are moving a state developmental center or at risk of moving into an institution. A portion of each day would be spent in the community.

Project Requirements:

The successful bidder must have prior experience in successfully providing services for client moving from the state developmental center.

The program services must include intensive interventions supported by consultants who will provide creative and proactive services required to implement the proposed program. The consultants will be expected to conduct assessments and provide training to direct care staff.

The day activity site must have one (1) full-time program director whose minimum qualifications must include a bachelor’s degree;

The program will also have one FTE specialist (nursing and/or behavioral may be primary needs), however this requirement is client driven and may be met by a combination of specialist hours.

The day site should have a minimum of 3,500 square feet.

Allowable Use of Funds

1 Start Up funding may be available. These funds may be utilized for site lease payments, rehabilitation, furnishings, supplies, adaptive equipment, staff training and related expenses.

Eligible Applicants

Proposals may be submitted by for profit or not-for-profit corporations. The applicant must have relevant experience and background. All partners involved with the project must sign the Applicant Information Form (Attachment B).

Board members and employees of Harbor Regional Center are not eligible to apply.

Evaluation Criteria

Each proposal will be evaluated based on but not limited to the following criteria:

Submission Process

The proposal (described below), Applicant Information form (Attachment B), Financial Information form (Attachment C) and Budget for Start-Up Costs (Attachment D)will be submitted for evaluation to the HRC/ RFP review team which will select candidates which best meet the specified criteria.

All submissions must be type-written in 12-point, Times New Roman or Arial font, on white 8 ½ x 11 inch paper, single-sided only. All pages should be numbered, and include an identifying footer with agency name. Please submit five (5) copies. Faxes will not be accepted.

The Project Proposal must include:

1. A description of prior relevant experience and credentials in serving clients who have moved from the state developmental center. 2. A description of the applicant’s demonstrated capacity to provide services in a linguistically and culturally competent manner. 3. A description of the applicant’s philosophy, values and vision statement specific to providing services to the targeted client population; 4. A description of history of success with developing, operating day activities; 5. A description of prior successful collaboration with Harbor Regional Center; 6. A description of services to be provided; 7. A description of the staff qualifications, consultants and salaries to be utilized; 8. The proposed monthly client activity schedule for both community and on-site; 9. A description of the possible client equipment; 10. The communication plan with caregivers; 11. A timeline for completion of the project; 12. The proposed start-up budget; and 2 13. Financial statement;
