Nicole Saunders & Tess Doward - Medicines Optimisation Technician, Luton Clinical Commissioning Group: Gave presentation regarding pharmacies ordering repeat medication on behalf of patients. Discussed audit carried out at a Luton surgery showing potential 2 million wastage if we continue to let pharmacists order on behalf of patients. Full briefing attached. After discussion group were asked whether they felt 1, we should continue as we are, 2, we stop pharmacies order where patient is capable of ordering themselves. 9 of the 10 people present agreed we should stop pharmacies ordering on behalf of patients unless the patient has given their order to the chemist. Further discussion regarding chemists not always being able to fulfil order. Tess confirmed that pharmacy should inform patient of this and when they will get further stock in before dispensing any of the prescription so the patient can go to an alternative pharmacy if they wish to.

What has happened in last 3 months:

New arrival: Congratulations to Norma Brown on the birth of her first grandchild, unfortunately Norma is not well and unable to attend today’s meeting. Group asked us to pass on congratulations to Norma.

Health Watch Survey: Met with Ray Kokabi healthwatch regarding funding for disabled access at front of building, NHS England have he is not able to negotiate on our behalf. I have to get 3 quotes for this, one builder unable to quote until plans drawn up. Have quote for plans to be drawn up, need one more quote to send to NHS England with application, but funding is limited so not sure this will be a priority.

Ethnicity Audit: of the 5489 we have recorded, 35% British, 29% Bangladeshi, 13% Pakistani, 5% Indian. (Asian added together 48%) 16% other.

Health watch report: Sent in by Diane Moles to look at, will put on PRG notice board.

Avoiding unplanned admissions/Care plans: New service, we have identified 2% of our adult population (18+) who we think are at risk of unplanned admission. Doctors have spoken to these patients and set up care plans. They have been coded to enable us to review.

1 Over 75’s: All patients over the age of 75 need to be informed of their registered doctor. We have written to all our patients and have now changed our letter so all new patients are informed of their named doctor when they join the practice.

Reducing/increasing sessions: Dr Campbell has reduced 2 sessions, Dr Varghese has increased 2 sessions. Dr Duffy plans to reduce 1 session on 1.9.14 and Dr Varghese will increase 1 session. We have therefore re arranged lists to make even pro rata. Patients can still see any doctor they choose.

Cluster Groups: We now have cluster groups instead of peer review groups. There are 4 groups in Luton all GP surgeries are split and belong to one of the 4 groups, most are formed from the original peer review groups. These groups are green or blue and also include community nurses. We are one of 7 practices, Barton Hill, Gardenia Avenue, Medici, The Moakes, Whipperley & Woodlands, Avenue. Working with CCG to look at avoiding admissions and other projects they may want to do in Luton.

A&E Attendance: We were asked if we have seen a reduction in A&E attendance since we have been offering late night appointments, it was confirmed no significant change has been seen.

Feedback from patients and CCG Meeting:

Women & Children’s groups: Discussed at CCG meeting, Maureen Mathews wanted feedback to take back to meeting. It was agreed that it may be useful for one group arranged by CCG may be of benefit, but not individual groups at each surgery.

Patients were thanked for attending the meeting and for items suggested for agenda. Anyone who has any other suggestion to add to agenda, please forward to Yvonne at any time. They will be held on file until the next meeting.
