HWSE Online Benefit Enrollment System How to Enroll in Benefits During Open Enrollment Open Enrollment Begins November 3 and ends November 24, 2014 To Log In: Log directly onto the ADP Employee Self Service Portal at https://portal.adp.com and go to Benefits > Health & Welfare. OR, if you need assistance logging onto the Portal, go to the CCSNH Benefits web site at http://www.ccsnh.edu/about-ccsnh/human-resources click on ‘Health and Welfare Benefit System - Online Benefit Enrollment System’ and follow the step by step instructions.

On the Portal Welcome page, you will find news and benefit information. To access your benefit options and perform your Open Enrollment event click on Health and Welfare.

2015 Open Enrollment Instructions November 3 – November 24, 2014 The Home Page Once you log onto the HWSE site, you’ll be able to Review your Current Elections; Access the Document Library; Manage Beneficiary Information; Access the CCSNH Website and; Review Job Aides which will assist you in the elections of your benefits To begin your 2015 enrollment process click on Enroll Today!

1 The Summary Page (Review / Change Benefits)

On this page, you can access your current elections and will be able to make any changes to your benefit elections.

Simply click on the Benefit area you would like to make a change to.

Please note: If you want to ADD a dependent, please click on Manage Dependents and add your dependent.

2 3 Medical Plans (Blue Choice, National PPO, and Medical Buy Out)

Once you’ve clicked on the Medical benefit area, you can enroll in either the HRA BlueChoice New England Plan OR the HRA National PPO plan.

If you have other non-CCSNH insurance, you can “Waive Coverage” for both insurance plans and elect the Medical Buy-Out plan.

Once you have made your decision, click Done at the bottom of the page.

Please Note: When selecting Coverage, your eligible dependents(s) will appear in the Coverage Information section below your election. Checked boxes indicate that you are requesting they will be covered under this plan choice.

If you no longer want to cover a dependent, simply uncheck the box beside the dependents name.

4 Pharmacy and HRA

Both are considered “bundled benefits” and will automatically be elected with a medical election (view only). There is no need to do anything Dental Plan

Here you can enroll in or waive coverage.

Click Done.

5 Enroll in Additional Benefits

The following pages allow you to enroll in additional CCSNH benefits:  Voluntary Vision.  Optional/Voluntary Life for Employee, Spouse and Child (Subject to Evidence of Insurability).  FSA Plans for Dependent Care and Medical care.

Note: Basic Life and AD&D and Long Term Disability programs are benefits provided by CCSNH, you cannot make any changes to these plans.

Click the Done button at the bottom of each page when you have made your elections. 2015 Medical Buy Out (MBO) Plan Enrollment Reminder: You must re-enroll for 2015; your 2014 elections will NOT carry forward

6 Medical Buy Out Plan

If you have non-CCSNH Medical Insurance, you simply need to “Waive Coverage” under Medical. Then proceed to the Medical Buy-Out (MBO) page and select Medical Buy Out.

Make sure you elect the dependent(s) who would be eligible for healthcare coverage, so that you receive the appropriate incentive payment.

Please Remember! The MBO does NOT carry over from the previous plan year. If you want to elect this benefit again for Plan Year 2015 (or for the first time), you MUST make the election again during Open Enrollment.

Also, you MUST complete a new Enrollment Form (which can be found in the document library and provide CCSNH with proof of other insurance.

Click Done.

DeltaVision – Voluntary Vision Program Reminder: You must re-enroll for 2015; your 2014 elections will NOT carry forward

7 DeltaVision Plan

On this page, enroll in the Voluntary DeltaVision Plan.

Please Remember! The DeltaVision Plan does NOT carry over from the previous plan year. If you want to participate in this benefit plan again (or for the first time), you MUST make the election during Open Enrollment.

You decide on what coverage you want. Employee only, Employee +1 or the Family Plan.

Click Done.

2015 Medical and Dependent Care FSA Plan Enrollment

8 Reminder: You must re-enroll for 2015; your 2014 elections will NOT carry forward FSA – 2015 Medical Reimbursement Plan

On this page, enroll in the 2015 Medical FSA Plan.

Please Remember! The FSA Plan does NOT carry over from the previous plan year. If you want to participate in this benefit plan again (or for the first time), you MUST make the election during Open Enrollment.

Update! The annual contribution amount for Plan Year 2015 has increased to $2,550.00.

Click Done. Flex Administrative Fee

All participants, in the Flex Medical Account, will be responsible for an administrative fee of $3.65 per month. This fee will automatically be applied to your election; there is no need for you to do anything.

9 FSA – 2015 Dependent Care Reimbursement Plan

On this page, enroll in the 2015 Dependent Care FSA Plan.

Please Remember! The FSA Plan does NOT carry over from the previous plan year. If you want to participate in this benefit plan again (or for the first time), you MUST make the election during Open Enrollment.

Simply select the plan, enter the amount of your 2015 annual enrollment, and press Calculate Cost.

Click Done. Flex Administrative Fee

All participants, in the Flex Dependent Care Account, will be responsible for a monthly administrative fee of $3.65 per month. The fee will automatically be applied to your election; there is no need for you to do anything.

10 Beneficiary (ies) Designation

If you want to designate your beneficiaries online, for either your Basic Life or your Additional Voluntary Life follow the steps below:

Click on the life insurance plan (i.e., Basic Life and AD&D);

Click on Add Designation; Choose either the dependent listed OR add one.

Enter in Percentage and Designation and click Done.

Last Steps!

Important! Once you have made all your benefit elections be sure to click Confirm Elections.

11 You’re Almost There!

Certification Statement and Email Address

Click I Agree on the Certification Statement

Choose your email notification preference and click Submit.

Confirmation

Once you press the above Submit, you will be brought to a Confirmation page.

You will see a blue bar notifying you that your transactions are being processed.

Do Not Log Out Yet!

12 PLEASE MAKE SURE YOU GET A GREEN CONFIRMATION BAR WITH A CONFIRMATION NUMBER before you click Done at the bottom of the page.

Your Enrollment is Complete!

Once you click Done, you will be brought back to the Home Page. Here you can see that your Open Enrollment elections are confirmed and you can view your elections by selecting Annual Enrollment – 1/1/2015 from the Benefits Overview drop down box.
