Dear Resident:

In an effort to maintain a high standard of resident services, Management’s policy is to enhance apartment units for residents who have been in continuous occupancy for an extended period.

Management has determined that you are eligible for Painting Carpeting Other ______enhancements. However the following conditions must be observed: 1. Management is not responsible for personal property. 2. Walls must be original color (only one coat of paint is provided) 3. Items beyond normal wear and tear may incur time and materials charges 4. You are a resident in good standing. 5. You must stop by the Rental Office within the next 30 days and sign a form agreeing to this service. Management will then comply as soon as scheduling permits. If you do not want to have the above enhancement performed in your unit, please sign below and return this form to the Rental Office. After 30 days if you have not contacted Management, Management must resend this form to you for you to decline or accept the enhancement. 6. Under the government approved management plan, Management is required to provide the above offered enhancement. Unless a reasonable accommodation situation exists, Management will perform this enhancement. If this situation exists, please discuss it with Management.

Painting 1. remove all items hanging on the walls 2. store knick-knacks/breakables away 3. move all items away from the walls as directed by Rental Office

Carpeting 1. store knick-knacks/breakables away 2. put all loose items inside drawers. 3. clear floor area of any remaining small/loose items

If I do not properly prepare for the enhancements scheduled and therefore the contractor is not able to perform the services, I understand and agree to pay a fee of $35.00 and further understand that a new request must be submitted for the service.

I/We hold TM Associates Management, Inc., the Agent for (insert property name here) and all it’s subcontractors harmless against any claim or liability for damage to such personal property or effects.
