Puppy/Dog Ownership Agreement

Helvey Buyer/New Owner: ______Address: ______City: ______State: _____ Zip ______Phone: ______Said puppy/dog is being sold/bartered/given away under the following conditions:

1. The puppy/dog shall not be used for breeding of any kind. 2. The puppy/dog shall be spayed/neutered by 6 months of age or within 30 days of taking possession of dog, whichever date is later. a. AKC registration papers and/or AKC puppy registration application will be held until buyer has supplied breeder with a veterinary spay/neuter certificate. 3. The puppy/dog shall not be sold, leased, bartered, given away or donated to another party(ies) without the express written consent of Cindy L. Helvey. 4. The puppy/dog shall never be surrendered to a shelter or rescue organization. 5. The puppy/dog shall not be abandoned or allowed to run free at any time. 6. The puppy/dog shall be kept as a pet and will be maintained in appropriate conditions for its health and welfare:  Will receive quality diet suitable for its age and condition – NO Science Diet! Recommended foods include: Pro Plan - Performance and adult/puppy Chicken and Rice formulas. Eukanuba - Performance or Small and Medium Breed regular formulas – no lamb/rice or other specialty varieties. Solid Gold Hund-n-Flocken - puppy/adult. Recommend feeding this food with Solid Gold Seameal Powder w/ Flaxseed Meal. The Missing Link – only nutritional supplement I recommend other than the one listed above.  Will receive minimum yearly veterinary medical exam and on-going veterinary care to include: Necessary vaccinations and worming. Initial _____ Preventive parasite control – heartworm, flea, tick, etc. Dental care and cleaning as needed. Emergency or other veterinary care as necessary.  Will receive regular grooming and basic hygiene care to include: Brushing of coat as needed to prevent matting and to keep coat free of foreign matter. Cleaning of ears as necessary to prevent infection. Bathing as needed to keep skin healthy and free of irritation. Clipping of hair on regular basis (2-6 week intervals) as needed to prevent matting and skin irritation.  Will receive training and socialization as needed to be a well-mannered house dog that is reliable and friendly in public situations. Recommended training includes: 1. Puppy Kindergarten classes where appropriate. 2. Basic Obedience course. 3. Crate training for bodily function control and for confinement when unsupervised. 4. House-training for manners and proper behavior with people and possessions. Socialization includes: 1. Training courses for interaction with other people and animals. 2. Interaction with family members and guests in the home. 3. Taking the puppy/dog on walks or visits where he/she can interact with strangers in a positive, non-threatening manner.

If Buyer/New Owner should become unable to care for dog, or is unable to provide a reasonable home and/or necessary medical and grooming care, or is unable to meet other basic requirements of dog ownership, he/she/they agree to immediately notify Cindy L. Helvey and to surrender the dog to her, with all AKC and accompanying paperwork and health records, without promise of financial restitution or compensation.

Buyer/New Owner further understands and agrees that should Cindy L. Helvey discover that any part of this agreement has been violated or is not being performed as agreed, or if the puppy/dog is neglected, mal-treated, mal-nourished, or is not receiving adequate or necessary medical care, Cindy L. Helvey shall have the right to repossess said dog and all AKC and accompanying paperwork necessary to transfer ownership to Cindy L. Helvey, without financial restitution or compensation.
