ATTENDANCE REQUIREMENT ---Attend all group supervision classes. Instructors Alicia Hoerner and Paul Caldarella will visit on site twice this semester. Additional visits may be completed as needed.

Presentation 1) Consult with your supervisor and research available resources for information on district, state, and federal policies on your topic. 2) Prepare brief written outline to share with class highlighting the purpose, time lines, regulations, and how to carry out this of this aspect of the process. 3) Discuss anecdotal suggestion from your supervisor on how the process is addressed in their building, tips, trouble spots, etc. 4) Include samples of necessary paperwork.

Also, prepare an inspirational/spiritual thought and offer the prayer on the same day. January 30 Goals Use goal-setting chart from practicum portfolio template to set at least two goals for the semester. The activities and indications of success should be measurable and observable. January Time Log Complete practicum time log through end of January. February 18 Group/Individual Complete portfolio template notes for group and/or individual counseling cases at your practicum site. You need one group and one Counseling Notes individual case of 6-8 weeks for your entire practicum year. If you have either in your portfolio from Fall semester this is not necessary to do again. Make sure you formulate the problem, intervention, and how you will collect and use data. You will need to address your projected plan when you turn in your final notes in April. Peer reviewed #1 Turn in one psych report with one peer review using the Assessment Report: Evaluation Form in the portfolio template. Psych Report February 28 Finalized #1 Psych Turn in final psych report with corrections from peer review. Report Demographics Use demographics form in practicum portfolio template to access the USOE website to obtain specific demographic info for you school(s). Professional Provide professional disposition form from practicum portfolio template to your supervisor, to complete and review with you. Please turn in Disposition to me in electronic form and save a copy for your portfolio. Mid-Term 10 Provide mid-semester evaluation form from practicum portfolio template to your supervisor, to complete and review with you. Please turn in Domain Evaluation to me in electronic form and save a copy for your portfolio. February Time Log Complete practicum time log through end of February. March 18 SOAP Counseling Use SOAP form to take thorough counseling notes for ONE counseling session with any student. Notes Goal Update On the goal chart template that you turned in to me in January, please fill in fall-mid semester update at the bottom of the chart and submit to me. Describe the progress you have made on the goal and what you still need to do to complete the goal. List of Assessments Continue to add to your assessment chart from the fall portfolio to track the type of assessments you have completed, with a description of the test, what it measures, and why you would use that specific test, then tally the number of times you have administered each specific assessment. Ethical Dilema Use ethical dilemma template to address an ethical problem you have observed or experienced at your practicum site. The steps will be reviewed in class. Eval of Supervisor Use the evaluation of supervisor in the practicum portfolio template to evaluate your supervisor and turn in to me in electronic form and save a copy for your portfolio. Supervisor’s Provide the On-Site Supervisor’s Evaluation of School Psychology Practicum Students form in the practicum portfolio template to have your Evaluation supervisor complete and review with you and turn in to me in electronic form and save a copy for your portfolio. April 18 April Time Log Complete practicum time log through end of April. Portfolio Compile all fall and winter assignments and forms into the practicum portfolio template and also summarize bullet points of activities you =NASP 10 domains have had in each the 10 domains for fall and winter.

Child Abuse Reporting: Students and supervisors working in the schools have an ethical obligation to promote student wellbeing and a legal obligation to report suspected child abuse. Reporting suspected child abuse should be done in collaboration with K-12 school administration. BYU students should avoid taking on an investigative role and let the school and state officials work on substantiation. When in Utah, reports are made through Child & Family Services (Child Abuse Intake Hotline: 1-855-323-3237), or when outside Utah, contact the equivalent state office or local police.

BYU Students should document the date, time, and the name of the person that took the call to Child and Family Services. If a student is in a situation where they suspect child abuse, but the school administration or the school-based supervisor do not believe a report is justified, the student should immediately contact their BYU supervisor for a consultation.
