has partnered with HSA Bank to offer a Health Saving Account (HSA) to accompany your health coverage, .

An HSA is a tax-favored savings account that is used to pay for qualifying medical expenses such as deductibles and other out-of pocket expenses. Unlike other medical accounts, your HSA funds roll over from year to year. After age 65, you can withdraw your HSA funds from the account without restrictions. Visit www.hsabank.com to learn more about HSAs.

Enrolling in an HSA is easy. Simply sign on to the secure online application portal at https://secure.hsabank.com/group_enrollment/enrollment.aspx?id=XXXXXXXXX. Or you can complete and mail the enclosed Health Savings Account Application and Eligibility Form to HSA Bank, P.O. 939, Sheboygan, WI 53082. Once your application has been processed, HSA Bank will establish your account and you can expect to receive the following in approximately 7-10 business days:

1. Welcome Kit: Includes your account number and important account details. 2. Debit cards: (If ordered) Once you receive the card, you will need to activate it by calling the toll- free number provided on the card. You will also select a PIN during card activation. If you ordered a card for your Authorized Signer, it will arrive separately in that person’s name. 3. Checks: (If ordered) There will be 50 duplicate checks and 10 deposit slips with your order.

Please note that if you do not open an HSA, we will be unable to facilitate pre-tax contributions into your HSA, thus limiting the valuable tax savings you receive.
