MASTERS EXTERNSHIP FINAL SELF-EVALAUTION MASTERS EXTERNSHIP FINAL SELF-EVALUATION

Instructions: You must complete this document and share it with your Field Mentor at the conclusion of the Externship. After your Mentor reviews the document, you and your Mentor should meet to discuss the Externship experience, your progress towards Learning Plan goals, feedback, and any other issues or questions. Following this meeting, your Mentor must sign your evaluation and you must share a copy with your Faculty Supervisor. You should also reflect on the final evaluation meeting in your journal. This form is in word document format so you can type into it directly and use the space you require.

Part 1: Progress towards Learning Plan Goals

A. Evaluate progress towards goals established in Learning Plan (list each goal, steps taken to meet them, and progress since the midterm evaluation).

B. Identify any issues that may have hindered your ability to make further progress towards any of your Learning Plan goals.

C. Identify ways in which your Mentor and/or Faculty Supervisor helped you continue to work towards your goals since the Midterm Evaluation.

Part 2: Academic Experience

A. Describe how the Externship has continued to contribute to your MELP, MERL, or LLM education since the Midterm Evaluation.

Part 3: Professional Experience

A. Describe how the Externship has continued to contribute to your professional skills and behavior (i.e. writing and research ability, professional responsibility and ethics, organization and efficiency, etc.) since the Midterm Evaluation.

Part 4: Summary of Overall Performance

Evaluate yourself, focusing on areas of your strengths, areas where you may need improvement.
