Community Investment 2016 Year-End/Outcomes Report On the 2015 Request for Funding (RFP) Program Application:  Question #5 stated- “List the action steps, timeline and resulting outcomes for this program.” And  Question #6 stated- “Describe how the desired outcomes will be measured and what evaluation tools/methods will be used. Please include an evaluation timeline.” Please provide your agency’s results for the 2016 year below.

Outcomes for 2016: 1. 28 new volunteers trained 2. 86 new children assigned to a CASA volunteer 3. Total number of children served by CASAs: 298 4. Average length of time a case was open 24.5 months 5. 1,025 children in foster care 6. CASA volunteer hours: 3,814 7. Personal Advocacy Services Provided: 7,166 8. Number of CASA assigned cases closed whose cases reopened: 0 9. Number of children who experienced abuse/neglect while in foster care: 0 10. Number of CASA closed cases: 117 Results: Adoption: 47 Reunification: 24 Child aged out: 1 Guardianship: 14 Long Term Foster: 3 Long Term Relatives: 2 Other: 26
