Perhaps unique among the uniformed services, Commissioned Corps officers are spread out nationwide and are immersed in day jobs in various capacities yet must quickly coalesce at deployment sites and rapidly reach operational efficiency. Our recent deployment experience, which began frantically, confirmed what CDR Weitzman and I had long suspected—that despite distance and unfamiliarity with one another, Commissioned Corps officers display an incredible sense of teamwork and camaraderie when it counts the most. Such teamwork began shortly after being rostered the morning of March 12, 2020 with a call to CDR Weitzman regarding our pending same-day deployment. Each of us had a challenge. This was to be my first deployment outside the DC-Maryland-Virginia area, and I was uncertain as to what I needed to bring with me or how the process worked. CDR Weitzman, who had been teleworking that day, wanted several CDR Beverly Weitzman spare items that had been left near her office an hour away from her home. We both sprang into action. CDR Weitzman and other experienced officers advised me on how to prepare and what to expect. Meanwhile, I picked up some of the spare items for CDR Weitzman, saving her a two-hour roundtrip drive. CDR Weitzman received her orders and immediately notified me. I realized we would be deployed to different sites but still departing from the same airport around the same time. By the time I arrived at the airport, CDR Weitzman had already passed through security. She circled back to the concourse where I handed her the items she had wanted and, with little time to spare, re-entered security and boarded her flight. Unfortunately, I missed my flight and was rebooked for the following morning. Once each of us arrived at our destinations, the spirit of cooperation and collaboration was consistently evident. Officers who were unclear about where to report to, or who lacked transportation, were given instruction or rides by fellow officers. On base, swab team members and case managers worked together to train and help each other. You know, as you get to the first 50 and 75%, it gets harder and harder to make that reduction as you move forward, so it’ll probably take us another five years to get from 75 to 90%. On behalf of the sickle cell disease community and the roughly 100,000 individuals living with sickle cell in the United States, we are grateful to be able to provide policy recommendations for consideration in the roll-out of COVID-19 and other public health support immediately, and throughout your administration. Sickle cell disease (SCD) is an inherited blood disorder that disproportionately affects Black and Brown populations in the United States, including roughly 1 in 365 Black and African Americans, and 1 in 14,000 Hispanic Americans. Due to racism and patterns of health inequities in the United States, the SCD population has been marginalized in the realms of research, data collection, education, and access to quality care across the healthcare continuum. Without robust funding for research and treatment, the lives of those living with SCD are approximately 40 years shorter than the average U.S. adult lifespan. Furthermore, the lack of coordination and proper preventative care provided by the medical system results in roughly $2.98 billion in annual healthcare costs. The COVID-19 health crisis has only exacerbated the disparities and inequities found in the SCD community.     October 15, 2020 Robert Redfield, MD Director Centers for Disease Control and Prevention Dear Dr. Redfield: On June 23, 2020, the Association of State and Territorial Health Officials (ASTHO), the National Association of County and City Health Officials (NACCHO), the Association of Immunization Managers (AIM), and the American Immunization Registry Association (AIRA) sent a letter offering our support, expertise, experience, and partnership for planning and executing a successful COVID-19 vaccine program. The letter also included our feedback and emphasis on the importance of building on the existing vaccine delivery infrastructure. The immunization community recognizes the critical importance of CDC, Operation Warp Speed (OWS), and other federal partners’ having timely and complete access to data on doses distributed and administered. These same needs exist in each state and local jurisdiction where state health officers, governors, legislatures, immunization programs, and state-based COVID- 19 task forces will need to closely track vaccine distribution and administration to carry out their mission of protecting their populations’ health. As organizations that work closely with states and local jurisdictions that will be on the front lines of vaccine distribution and administration, we’ve seen positive movement, such as setting vaccination priorities and decisions to use existing, centralized vaccine distribution infrastructure. Thank you for acting on these key issues. However, there continue to be areas of concern to the immunization community. The attached table outlines our concerns and recommendations. We believe these recommendations are in line with CDC’s COVID Planning Assumptions, COVID Scenarios, and Early COVID Action Items Checklist. Successful vaccine rollout in this country hinges on immediate and clear guidance that leverages the current health information technology infrastructure used nationally by both private and public health care sectors.

The number of new daily cases surpassed 200,000 for the first time on Friday and the total number of infections in the United States reached 13 million. On Sunday, the country reported more than 100,000 new coronavirus cases for the 27th consecutive day and 21 states recorded records for hospitalizations. Trump Administration • Data from the Transportation Security Administration reveals that in the days before and after the Thanksgiving holiday, between 800,000 and one million people passed through T.S.A. checkpoints each day. • Dr. Deborah Birx, the coordinator of the White House Coronavirus Task Force, said Sunday that people who traveled for Thanksgiving “have to assume you were exposed and you became infected,” and encouraged travelers to get tested in the next week and to avoid high-risk individuals. • Italy will open its borders to flights from the U.S. without a quarantine mandate for the first time since the imposition of Covid-19 travel curbs. The waiver, backed by rigorous testing before departure and on arrival, is the first on a trans-Atlantic route, according to Delta Air Lines, whose flights from Atlanta to Rome Fiumicino will trial the new rules. • An administration official confirmed last week that the White House is considering lifting restrictions on travel from the European Union and the United Kingdom.

Azar II Secretary United States Department of Health and Human Services 200 Independence Avenue, SW Washington, DC 20201 Dear Secretary Azar, As organizations representing health care providers, patients, public health, scientists, and industry, we are writing to alert you to the latest significant evidence that the antibiotic market is in jeopardy —the December 27, 2019 Chapter 11 bankruptcy filing and restructuring of Melinta Therapeutics —a biotech company with four products on the market. While Melinta’s products remain available during this process, this is yet another sign that will drive investors away from this space. We implore you to take swift action to stabilize and strengthen the antibiotics market and make the investments necessary for a comprehensive response to antimicrobial resistance, including stewardship, diagnostics, surveillance, prevention and research. The antibiotics market is in a state of crisis. Melinta is not an outlier—it is the second small antibiotics company, following Achaogen, to file for bankruptcy in 2019. The stock prices of the remaining small antibiotics companies with approved antibiotics make clear that additional bankruptcies are likely in 2020 unless federal action is taken. Small companies are particularly important in the antibiotic space as they are developing the majority of new medicines to address antimicrobial resistance (AMR). Many of these companies are struggling to secure private investment to bring these medicines to the market and patients. Additional failures of companies following successful approval will sap what little investment remains in this space with catastrophic consequences on an already inadequate pipeline. Allowing the antibiotics market to collapse endangers modern medicine, public health and national security.
