OSHA OUTREACH TRAINING PROGRAM FACT SHEET NOTICE – As of October 1, 2003, outreach trainers will send requests for 10- and 30-hour General Industry and Construction, 10- and 30-hour Maritime, and Disaster Site Worker student cards to the OSHA Training Institute Education Center where they last took their trainer course (500, 501, 502, 503, 5400, or OTI 5600). WHEN: · Send your request to the Georgia Tech OSHA Training Institute Education Center (OTIEC) within 3 months of completion of training. NOTE – Trainers who took their last general industry and/or construction trainer courses at a different Education Center will have to send card requests to the respective OTIEC. WHERE: · Submit requests for student cards to: Georgia Tech OSHA Training Institute Education Center ATTN: Lauren Duffey GTRI/ELSYS 260 Fourteenth Street, NW; Room 422 Atlanta, GA 30332-0837 · Requests will be accepted via mail, fax, or email (no e-mail attachments please – all information should be in the body of the e-mail message). . Fax: 404/407-9024 ▪ E-mail: [email protected] N OTE – If you have not been assigned an ID number by GA Tech, please mail in your first request and include a copy of the 8.5 x 11” certificate you received when you completed your last trainer or update course with Georgia Tech AND leave the space for ID number blank. HOW THE PROGRAM ADMINISTRATION WILL WORK: · Each request requires the same information: 1. OSHA Outreach Training Program Report (All card requests must include a start date and end date on the program report form in the date section.); 2. Student names; 3. List of course topics and time spent on each topic; and 4. A copy of your 8.5 x 11” trainer certificate, if this is your first request. · We will review requests for: - Trainer qualification (valid authorization status); - Statement of Certification - Compliance with the guidelines (topics, time, etc.); and - Completeness of information. Discrepancies will be reviewed with the trainer, and complete, valid requests will be filled within six weeks of receipt. · Refer to the Outreach Guidelines for additional information; the guidelines are also available at: http://www.osha.gov/dte/outreach/construction_generalindustry/general_industry.html QUESTIONS?
