I will be responsible for following the eligibility rules, my team’s conduct, and making sure my team appears at the tournament. I have informed my players that there are inherent risks involved in participating and that they should have health insurance to cover any possible injury.

Hold Harmless Agreement The following users will voluntarily participate in a North Carolina Collegiate Flag Football Tournament and use the Intramural Fields on the main campus of Appalachian State University, October 20-22, 2006. I understand the physical nature of the Flag Football Tournament and the substantial potential risks inherent in that activity, including risks of personal injury and death, and property damage or loss. I assume those risks, and I assume sole and full responsibility for my safety during participation in the Tournament and related activities. I also understand and acknowledge that Appalachian State University does not carry insurance covering persons participating in the Tournament, and that I and/or my parents or guardians are responsible for obtaining appropriate insurance to protect me and/or them with respect to the risks involved in my participation in the Tournament. I further agree as a “PARTICIPANT” that photographs, pictures, and videos taken or made by the “SPONSORS”, in connection with the “PARTICIPANT’S” involvement in the event, or a reproduction of the same, as well as the “PARTICIPANT’S” name, may in any manner be used by the “SPONSORS”. I warrant as a “PARTICIPANT” I am in good heath and have no physical condition that would prevent me from participating in the tournament.
