MEMORANDUM FOR HOME AND COMMUNITY SERVICES FIELD STAFF

Subject: Supervisor Case Review form

The Supervisor Case Review (SCR) form has been revised to modify the way in which Home and Community Services (HCS) tracks quality reviews. The Automation and Analysis Unit (AAU) will be developing a new Access database that will allow for tracking and trending of quality indicators throughout the state. When the SCR is completed, the Home and Community Services Area Supervisor will send a copy to the Section Administrator’s office for entry into the database. Reviews will be compiled according to each regional area’s individual team number.

Adult Protective Service Manual 1700.00 The Index has been updated to reflect the addition of the SCR. 1707.00 The Index of Forms has been revised to include the SCR. 1707.82 The revised SCR form and instructions have been added to the manual. SCR revisions allow for different reviewers to use the form and include: the addition of reviewer/worker signature line on each page, information about the Worker and participant on each page, a team number indicator, and an update to reflect the move of Office of Special Investigations to the Office of General Counsel (OGC).

Case Management Manual 100.00 The Forms Index has been updated to reflect the addition of the SCR. 155.00 The SCR and its instructions have been added to the forms section of the Case Management manual.
