Especially for families living in Jefferson and Shelby Counties without medical insurance and with low income (i.e.: qualify for Unemployment/Medicaid/Disability). Space is limited to the first 60 people on a first come, first serve basis including a designated number of spots reserved for the Hispanic outreach ministry of the church.

If you qualify and depending on outcome of vision services, bifocal glasses for adults are available for $75 and single vision glasses for $60 to cover cost of lens materials. Referral services are available for children.

Directions: Take I-65 to Homewood. Take the Oxmoor Rd Exit (#256A) toward Green Springs Hwy/Palisades/Homewood. Oxmoor Road becomes Palisades Blvd. Bear right onto Palisades Drive. Palisades Drive will become Oxmoor Road. Stay on Oxmoor roughly ½ mile. Make a left onto Dale Avenue between Dawson Baptist (left) and Trinity United Methodist Church (right). We will be in the recreation building, on the basement level. Look for the white Cahaba Valley Health Care van and follow CVHC signs.
