NEVADA GOVERNOR’S COUNCIL ON DEVELOPMENTAL DISABILITIES

GOVERNOR Notice of Funds Available Brian Sandoval October 1, 2016 through September 30, 2017

The Nevada Governor’s Council on Developmental Disabilities (NGCDD) is seeking proposals for grants to help meet the Council’s goals and objectives listed in the draft Five Year Plan for Federal Fiscal Year 2017-2021.

Background The Nevada Governor’s Council on Developmental Disabilities is established under NRS 232.320. The NGCDD is funded by the Administration on Intellectual/Developmental Disabilities (AIDD) under The Developmental Disabilities Assistance and Bill of Rights Act, most recently reauthorized in 2000, and matching funds appropriated by the Nevada Legislature.

Each State Developmental Disabilities Council is required to submit a five-year plan to the AIDD outlining intended use of federal funding allocated for its basic operational grant. Federal law mandates that the plan address some or all of the Areas of Emphasis established by Congress. The Areas of Emphasis are as follows:

Quality Assurance Child Care Early Intervention Education Employment Health Transportation Recreation Community Supports Housing

The NGCDD accomplishes its mandates through several different avenues. Some activities are carried out by Council staff; others are accomplished by partnering with other established agencies and organizations to address areas of mutual concern. Finally, the NGCDD identifies gaps in service delivery and provides sub-grants to develop and implement projects that address innovative ways to fill the gaps.

Definition Developmental Disability means a severe, chronic disability of an individual that—

*is attributable to mental or physical impairments or a combination of mental and physical impairments; *is manifested before age 22; *is likely to continue indefinitely.

This disability must result in substantial functional limitations in 3 or more of the following areas of major life activity: self-care; receptive and expressive language; learning; mobility; self-direction; capacity for independent living; economic self- sufficiency. In addition, reflect the individual's need for a combination and sequence of special, interdisciplinary, or generic services. Individual supports or other forms of assistance that are life-long or extended duration and are individually planned and coordinated. Council Initiatives

During this funding period, the NGCDD is specifically looking for projects to help meet the following goal and outcomes as outlined in the 2017-2021 NGCDD Five Year Plan. Grants are from (October 1, 2016 through September 30, 2017).

GOAL III: In conjunction with individuals with intellectual and developmental disabilities and community entities, develop and strengthen systems that improve quality of services and access to quality services and supports in their local communities.

Objective 3.1 Create one transportation system model that is sustainable and replicable and will serve people with I/DD across the State.

 Identify one or more of the following strategies that will be utilized in your project: 1) Develop and expand transportation service options. 2) Expand and promote the use of voucher programs for transportation services. 3) Work with transportation companies to adopt policies that allow for more flexibility for pick-ups and drop-offs of people with I/DD. 4) Maximize efforts for sustainable grants and alternate funding sources in order to expand transportation services.

Expected Outcome(s): Progression will be made in moving the State into expanded, sustainable, universally designed and accessible transportation through the development of a new system and/or expansion of the current transportation system.

GRANT FUNDS AVAILABLE FOR OBJECTIVE 3.1 (10-1-16 thru 9-30-17): Up to $60,000

(There may be grant continuation possibilities for the project that demonstrates progression in their first year of grant funding towards the creation of a transportation system model that is being utilized throughout Nevada by September 30, 2021)

Objective 3.3 Develop and/or strengthen a minimum of one system that improves quality of services and access to quality services and supports for individuals with I/DD in their local communities by working with agencies to bring awareness and training to public safety and emergency responders throughout the State of Nevada.

 Identify one or more of the following strategies that will be utilized in your project: 1) Develop a training curriculum that is adopted and used by Emergency Personnel across the State. 2) One public safety entity in the state of Nevada will create policy to conduct regular trainings on level of care and service to individuals with I/DD. 3) Trainings will be conducted to Public Safety Entities Emergency Personnel throughout Nevada to provide awareness for a higher level of care and service to individuals with I/DD.

Expected Outcome(s): Higher level of care and service will be provided to individuals with I/DD by Public Safety Entities through development of a program that provides awareness and training to Emergency Personnel across the State.

GRANT FUNDS AVAILABLE FOR OBJECTIVE 3.3 (10-1-16 thru 9-30-17): Up to $30,000

Objective 3.4 Annually educate a minimum of 100 persons with I/DD and their families to be able to make informed health choices throughout their lifespan.

 Identify one or more of the following strategies that will be utilized in your project: 1) Encourage and/or facilitate the provision of services through telehealth to improve options for persons with I/DD in rural areas to access more health related services. . TOTAL GRANT FUNDS AVAILABLE FOR STRATEGY #1 (10-1-16 thru 9-30-17): Up to $30,000 2) Develop communication strategies that support consumer awareness to help people understand their role in person centered planning. 3) Engage and inform consumers, caregivers and providers about a No Wrong Door system. 4) Facilitate accessible and appropriate information on healthcare resources to people with I/DD. . TOTAL GRANT FUNDS AVAILABLE FOR STRATEGIES #2, #3, #4 (10-1-16 thru 9-30-17): Up to $30,000

Expected Outcome(s): a) Telehealth as established through 2015 legislation will progress to enable people with I/DD in rural areas to access more health related services. b) People with I/DD will be provided educational information that will give them the ability to make decisions throughout their lifespan guided by informed choice. c) Persons with I/DD will receive accessible and appropriate information on available healthcare resources.

GRANT FUNDS AVAILABLE FOR OBJECTIVE 3.4 (10-1-16 thru 9-30-17): Up to $60,000. A project that incorporates all strategies could be eligible for the entire $60,000 grant award. General Conditions

 Eligible applicants include public and private corporations, government entities, state agencies, school districts, and 501(c)(3) non-profit organizations may apply. Individuals, partnerships and unincorporated entities are not eligible.  Grant is from (October 1, 2016 through September 30, 2017).  There is no expectation of renewal for subsequent years.  Grant funds may not be used for capitol expenditures or for durable equipment.  Funds may not be used to provide direct services (personal services provided to the public).  Grantees must provide quarterly progress and financial reports in the format provided by the NGCDD.  Projects must focus primarily on people with developmental disabilities, their families and their support networks.  Projects must be sustainable after NGCDD funding ceases.  Projects must be replicable.  The NGCDD reserves the right to withdraw the Request for Proposal and to accept or reject in whole or in part any or all proposals received, to require modification to any part of accepted proposals, including the budget and the amount requested, and to waive any irregularities in the proposed projects.  NGCDD decisions regarding grant awards are made by the Council and are final. Specific Requirements  All applicants must comply with the policies and procedures outlined in the Grant Procedures Manual provided by the Nevada Governor’s Council on Developmental Disabilities (NGCDD).  All grantees must assure that they will comply with regulations, guidelines and requirements as they relate to the application, acceptance and use of Federal funds for this federally assisted project. A copy of the assurances is included in the Grant Procedures Manual.  Grantees, if required, must be registered with the Nevada Secretary of State and licensed to do business in the State.  Grantees must not appear on the Federal Lists of Parties Excluded from Federal Procurement or Non-procurement Programs. (The list of disbarred parties can be found on the Internet site at http://epls.arnet.gov.  Applications must be submitted electronically on the DD Suite website www.ddsuite.org by 5:00 pm on July 29, 2016.  Attendance at the video-conference training session is mandatory for all parties planning to submit an application. o If it is completely impossible for you to attend one of the video-conference trainings, please contact Sherry Manning or Kari Horn (775) 684-8619 for alternate training options. o The training will be held at the Nevada Disability Advocacy and Law Center locations (NDALC) Reno, Las Vegas and Elko on June 30, 2016 at 1:30 p.m. and July 13, 2016 at 1:30 p.m.
