2015 GUIDELINES Purpose: The AAFP Foundation’s Senior Immunization Grant Awards will provide support to Family Medicine residency programs (FMRPs) to enhance residents’ quality improvement and/or community health training, while improving influenza and pneumococcal vaccine rates in patients age 65 and older during the 2015/16 flu season (September 2015 through March 2016).
Support for the 2015 Senior Immunization Grant Awards is provided by Pfizer Inc. Deadline to Apply: April 6, 2015. All applications must be received by 8pm Central Time, Monday, April 6th. Award recipients will be notified by June 8, 2015. Eligibility Requirements: Must be a Family Medicine residency program (FMRP). Residents must be involved in the development, implementation, and reporting of the proposed quality improvement and/or community-based project. If selected, the FMRP agrees to comply with reporting regulations and must be able to meet all Reporting and Presentation Deadlines, as outlined below. May not be a recipient of a 2014 Senior Immunization Grant Award Project Period runs from June 2015 to August 2016. Reporting and Presentation Deadlines: 990 financial information for entity receiving the grant award due by July 2, 2015 (form will be provided). Sunshine Act disclosure information due by July 2, 2015 (form will be provided). This grant award is subject to reporting under the National Physician Payment Transparency Program (Open Payments), a.k.a. Sunshine Act. At a minimum, the full name and 10-digit National Prescriber Identification (NPI ID) of the Program Director or project lead, as well as the address of the FMRP must be provided. This information will be disclosed to Pfizer, Inc and The Centers for Medicare and Medicaid Services (CMS). Work Plan for Project due August 15, 2015 (extracted from application or updated, if needed) Interim Report due December 15, 2015 Mid-Evaluation Conference Call in January 2016 Report of Results/Findings due May 5, 2016 Abstract for Poster Presentation due May 16, 2016 Poster Presentation by one or more residents at AAFP 2016 National Conference for Family Medicine Residents and Medical Students from July 28-30, 2016 How to Submit Your Application: Your application can be submitted by electronic mail or mail. All applications must be received by 8 pm Central Time, Monday, April 6th. Grant Award: Up to 6 FMRPs will be recognized with a Senior Immunization Grant Award. The $11,200 award, presented to the FMRP, will contain the following elements: $10,000 grant to be used only for costs directly related to increasing immunization rates, which may include medical supplies, equipment rental or purchase, software purchase or lease, patient education materials; patient incentives; communication expenses; and mileage expenses, if applicable. $1,200 travel scholarship will be provided to the FMRP to allow one or more residents to attend the AAFP 2016 National Conference of Family Medicine Residents and Medical Students, and present the results/findings via a poster presentation. Resident(s) presenter must register and will not have fees waived for National Conference. Preference will be given to projects that can: Achieve influenza and pneumococcal rates of ≥75% or show a 25% base rate improvement (e.g., 40% improved to 65%) for those age 65 and older; and
Provide service to those who are medically underserved.

Reminder: Senior Immunization Grant Awards are intended to recognize FMRPs whose residents implement and report on a quality improvement &/or community-based project that is designed to increase influenza and pneumococcal vaccination rates in patients who are age 65 and older, especially in underserved communities, during the 2015-2016 influenza season.

The Senior Immunization Grant Award totals $11,200, which includes a $10,000 grant provided to the FMRP whose innovative project is selected; and a $1,200 for a travel scholarship to allow one or more resident(s) to present their results at AAFP’s 2015 National Conference. Funding from the grant award may only be used for costs directly related to immunization projects and may include the cost of medical supplies, equipment rental or purchase, software purchase or lease, patient education materials, communication expenses, patient incentives/reimbursement, mileage/transportation, and costs associated with presenting results at AAFP’s 2015 National Conference. Only high-level budget reconciliation will be required for the Final Report.
