VitalSign6: Making Screening for Depression the Sixth Vital Sign

Dr. Madhukar H. Trivedi will present an update on UT South Western’s unique program to treat depression in primary care settings currently being used by over 60,000 patients in North Texas. Depression affects an estimated 20% of patients at some point in their lives and is far more common in patients with chronic diseases. Studies show that patients that get treatment are far more likely to recover and do better than those who do not.

VitalSign6: Making Screening for Depression the Sixth Vital Sign is an easy-to-use, comprehensive program for the identification and treatment of depression in primary care, pediatric and specialty clinics.

The program utilizes an innovative web-based iPad application to administer the Patient Health Questionnaire (PHQ) and Measurement-Based Care (MBC)systematic assessments to patients.

VitalSign6 aims to provide medical providers with the means to efficiently and effectively diagnose and manage depression; improving care for patients with depression through: . The implementation of routine, evidence-based depression screening, . The integration into standard practice of a scientifically validated, measurement-based care (MBC) program for the treatment of depression for patients who screen positive, and . Integrated clinical decision support. Patients complete the measures on an iPad during the triage process, thus making screening for depression the sixth vital sign after body temperature, pulse rate, respiration rate, blood pressure, and pain.

The concept of MBC for the treatment of depression includes the systematic assessment of depressive symptoms, antidepressant treatment side effects, and antidepressant treatment adherence at defined intervals using validated rating scales with the goal of reaching full remission.

Despite great progress in uncovering the underlying mechanisms of many diseases over the last several decades, modern medicine has just begun to understand the human brain—the body’s most complex system and medicine’s greatest mystery. The Center for Depression Research and Clinical Care addresses a critical need for the advanced treatment of patients with depression and mood disorders. There have been substantive advances over the last 30 years, but there is no universally effective treatment. Nearly two-thirds of patients with depression fail to achieve a full recovery.

UT Southwestern’s Center for Depression Research & Clinical Care (CDRC) is a nationally recognized center of excellence in the research and treatment of mood disorders, including depression and bipolar disorder. The Center works for the prevention and cure of depression by: . Translating the most promising new treatments for depression more quickly from the bench to the bedside and disseminating these treatments in the community through network partnerships; . Conducting innovative research to better understand the causes of the illness and to study prevention strategies, as well as to develop a foundation for better and more personalized treatments for individuals based on their clinical and neurobiological features; . Developing a state-of-the-art destination clinic for those patients with the most severe and resistant forms of depression; . Training the next generation of providers and researchers; and . Engaging the community to improve awareness and knowledge about depression and related disorders, as well as enhance clinical care.
