CONTRACT TO RENT NORTH PARK SOCCER FIELDS FOR GAMES ONLY Must fill out one form per game.

Soccer fields at North Park must be reserved one week in advance unless a field is already being reserved at the same time and the on-site supervisor can be contacted in time to let him/her know of an additional rental. If you do not give us one week notice to rent a field you must pay a two hour penalty fee plus the regular rental fee. You must give us one week cancellation notice to get a refund. To make any changes in a rental, you must pay a one hour penalty fee in addition to the regular rental fee.

Rental Group School Public, Private or Soccer Associations -we should get a certificate of insurance from the group.

Recreation League participates - we should get each individual that will play to sign a participate waiver.

All other Groups - we should get each individual that will play to sign a participate wavier.

These rules must be followed so that we are in compliance with our insurance liability.
