LHC MATCH SHEET 15-16 Date: Instructions on how to fill the match sheet Team: L1 L2 L3 L4 Vixens M1 M2 M3 M4 Badgers Vets Girls Boys u8 u9 u10 u11 u12 Please follow the instructions. Return a match sheet as early as possible to the Opposition Type: League Friendly Tournament Treasurer if a match is postponed, cancelled or abandoned. Pitch: H/A Result: Win Draw Loss 1. Fill in the date, identify the LHC team or tournament by circling or striking out the Home Teas provided Yes / No Tea Venue Bell/other appropriate entry. Name the opposition and the type of match. Home or away, state the pitch Match Other Init Name Expenses Comment or venue, enter the result and circle or strike out the win/loss/draw as appropriate. For Home (alphabetical order) fee Income matches, state if teas were provided and circle or state the venue where teas were provided. 2. Fill in the pitch. This is the only way to determine where the match was played, and the club is billed appropriately. If the match is cancelled or postponed, a match sheet must be sent to the Treasurer. Cancel teas, stating date and time when cancelled. 3. List the players in alphabetical order. Full surnames are required and initial (first

name if surnames and initial are identical. (Not ‘guest player’). Identify the goalkeeper and if they are using their own kit (gk own kit). 4. State the amount of match fee paid. If not an adult fee and if sub has not yet been paid, please state reason for reduced fee (1/2 match, injury, student, junior, etc). If paying outstanding match fees, just total up and insert in match fees column e.g. if paying two full match fees, put in £20.00. If more complicated, state which matches are being paid for or make a new line entry.

5. If match fee not paid, write in ‘owes’ (or £0.00 if overpaid in previous weeks and add a comment to that effect) in match fee column.. Do not leave blank. State reason if a full fee was not paid. If expenses are paid, there must be a match fee receipt before any payment can be made. 6. If donations are made to the club or other receipts such as tea and pitch contributions a b are made, write in the amount in ‘Other Income’ and state the reason in the ‘Comment’ field. Totals 7. Petrol is paid at 25p/mile. Use the table below to determine amount. Enter the petrol amount in the expense column and state ‘petrol’ in the Comment column. Similarly, oranges, pitch hire, toll bridge etc. 9. State name of LHC umpire(s) and amount of fee paid. (Think umpire record For Treasurer Use card). If no refund is paid, please indicate. Match Fees Teas Subs Pitch hire 10. State tea venue, state amount paid, include the receipt with the match sheet. 11. Total up the moneys, and pass money and match sheet to Treasurer as soon as Petrol Teas Umpire fee possible. Cheques are accepted, made payable to ‘Lansdown Hockey Club’. If expenses are greater than income, then use money from preceding or following week to pay the difference.

12. Please make sure that the sheet is clear, legible and that the calculations are correct. If necessary, do rough calculations on the reverse, not on the front of the sheet. word 1 Sep 15
