Dear Partner,

As a leader in the San Diego Community, we invite you to identify and nominate the unsung heroes in your community for a Molina Healthcare Community Champions Award. Created in 2006, the Community Champions honors individuals that inspire others through selflessness contributions that positively affect the health and well-being of others. The Community Champions Awards were named in honor of Molina Healthcare’s physician founder, Dr. C. David Molina.

Nominees for awards may be volunteers or members of a recognized organization who demonstrate selfless dedication to improving the quality of life in their community. Winners will be honored at a dinner and awards ceremony on Tuesday, January 24, 2011. In addition, a $1,000 grant will be awarded on behalf of each winner so that they can “pay it forward” to his or her designated community organization.

Please take a moment to consider nominating a community champion in one of the several categories. You will find all the information about the award categories, submission guidelines and nomination process attached to this letter. Nominations must be submitted by November 30, 2011.
