IMPORTANT! PLEASE NOTE that you can NOT submit the PHYSICAL EXAMINATION FORM online. It must be filled out by a physician and returned to the campus athletic coordinators OR the coach at your child’s campus. Physical forms are available at each high school or middle school campus.

STEP 1.You will need to have the following information available to submit online appropriately:

 Your child’s ID number

 Emergency contact phone numbers

 Name and contact of primary physician

 Insurance policy information (if possible)

STEP 2. To access the required athletic participation forms for Killeen ISD, hold your cursor over the “Electronic Participation Forms” tab and left click. You will get a drop down list of all required forms. Click on the Online Submission Forms Tab.

STEP 3. To begin, there are 3 Tabs: Instructions, Physical/Medical History and Electronic Participation Forms. Be sure to click on the Instructions tab and read the instructions carefully. We will hand each student a hard copy of the Physical/Medical History form. If they loose it, you may print from the link.

Under the Electronic Participation Forms tab there are 3 sub tab forms to be completed:

 KISD Student-Athlete Information & Emergency Treatment

STEP 4. SIGN the DOCUMENT(S) To sign each document:

 Click inside the signature box and hold your mouse down,

 While holding down the mouse “sign” your name in the signature box. If you make a mistake and need to start over click on the refresh icon next to the signature box.

If you are using an IPAD, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.

REPEAT STEPS 1-4 FOR ALL REQUIRED DOCUMENTS SHOWN

IMPORTANT! To submit online appropriately, you AND your child MUST fill out and sign in ALL REQUIRED SPACES. Forms will not be accepted electronically without everything filled out by the student athlete and you the parent.

Once you have filled out all of the information on each page you will have the opportunity to print the document. You will receive confirmation email once the document(s) have been reviewed by the KISD Athletic Trainers OR your child’s campus sport coach.
