[579] 2 1 doctor, nurse practitioner, or physician’s assistant; pharmacist; 2 nurse; dietitian; and others; and 3 4 Whereas, diabetes management includes healthy eating, being 5 physically active, healthy coping and stress management, 6 medication management along with selfmonitored blood glucose 7 levels, as well as means to prevent complications, increase 8 productivity, and enhance quality of life; and 9 10 Whereas, diabetes is a challenging disease that affects the entire 11 family in many ways; therefore, family support is very important; 12 and 13 14 Whereas, people who are obese or overweight need to strive for 15 their recommended bodyweight range, which will improve their 16 overall diabetes management, bloodpressure control, lipid 17 management, and related outcomes; and 18 19 Whereas, comprehensive medication management (CMM) is an 20 essential component of coordinated care for the diabetic patient, 21 which ensures each patient’s medications are individually assessed 22 to determine that the patient is willing and able to take the 23 medication as prescribed and that the medication is appropriate, 24 effective for the medical condition, and safe in regard to the 25 patient’s comorbidities and in relation to other medications the 26 patient is taking; and 27 28 Whereas, CMM involves regular interaction between the patient 29 and the health care provider to ensure that as a team they are 30 meeting their clinical goals of therapy, with the prescriber having 31 ultimate decisionmaking authority for any changes made to the 32 patient’s medication or treatment regimen; and 33 34 Whereas, policymakers and the public should recognize that 35 medications and improved selfmanagement by the person with 36 diabetes are important prevention and treatment tools for patients 37 and their providers; and 38 39 Whereas, South Carolina policymakers should encourage the use 40 of CMM for highrisk patients, such as Medicaid patients who have 41 diabetes; and 42

[579] 3 1 Whereas, the South Carolina Senate encourages all South 2 Carolinians to reduce their risks of developing diabetes and 3 promotes the “American Diabetes Association (ADA) Alert Day,” 4 which serves as a wakeup call and asks all Americans to take the 5 Diabetes Risk Test to find out if they are at risk for developing 6 type 2 diabetes; and 7 8 Whereas, the South Carolina Senate supports awareness about the 9 prevention, education, treatment, and undertreatment of diabetes, 10 including the benefits of CMM, diabetes selfmanagement 11 education, and improved quality of care for persons at risk for 12 diabetes, as well as those presently living with diabetes. Now, 13 therefore, 14 15 Be it resolved by the Senate: 16 17 That the South Carolina Senate, by this resolution, recognize the 18 devastating effects of diabetes and proclaim Wednesday, March 19 25, 2015, as “Diabetes Awareness Day” in South Carolina, 20 coinciding with South Carolina’s “Diabetes Under the Dome 21 Day,” sponsored by the Diabetes Initiative of South Carolina and 22 the American Diabetes Association. 23 XX 24
