Brent Council and NHS Brent Clinical Commissioning Group (CCG) are pursuing a far-reaching transformation agenda that better meets the needs of our community. The CCG and the Council vision is to improve the quality of health care for local people and commission high quality service provision to meet local needs. We have a young, dynamic and ethnically diverse population, which is relatively deprived. NHS Brent Clinical Commissioning Group (CCG) comprises 66 member practices which are organised into five localities: We are one of the largest CCGs in North West London, meeting the needs of circa 350,000 registered patients. The CCG is responsible for the planning, organising and purchasing of £450m worth of healthcare locally.

The CCG commissions healthcare activity from a wide range of providers – the majority of acute Hospital activity is undertaken at London North West Healthcare NHS Trust, for which we are the host commissioner and Imperial College Hospitals. The major provider of mental health services is Central North West London Hospital and the major provider of community services is London North West Healthcare NHS Trust - Brent Community Services.

Over recent years, Brent Council has undergone major transformation, and this continues with a programme of unprecedented structural and organisational change designed to meet the financial challenges facing the public sector. We remain committed however to deliver high quality services while driving efficiency and cutting waste and bureaucracy and to driving forward service excellence, urban regeneration and community cohesion whilst recognising that we cannot ignore the financial reality of providing public services in this challenging financial climate. The Council has recently brought together adult social care, housing, public health and cultural services into a single Community Wellbeing Department, with the aim of improving health and well-being outcomes for our residents, particularly better targeting resources towards support for vulnerable people.

Leader ship This is a joint post working across health and social care and the individual will lead the delivery of the Brent Integrated Care transformation priorities on behalf of Brent. This will include the Better Care Fund (BCF) programme and the Sustainability and Transformation Plan (STP), using these schemes as a vehicle to achieve service transformation and integration.

Specifically the role is to;

 Support the CCG and Council in driving transformation and closer integration, as well as value for money in planning, commissioning, programme management and service delivery.   To provide effective programme management and operational delivery of the STP and BCF programmes across Brent CCG, Brent Council and wider partners as necessary.

 Oversee delivery of integration projects, working with scheme leads to address barriers, problem solve and escalating as required to ensure that delays are managed and mitigated to remain on track.

 Ensure a robust performance management process is operational for all national and local indicators to enable evaluation of impact and performance of integration programmes.

 To support the development of key strategic relationships between the two organisations and wider senior partners in health, including the Health and Wellbeing Board, Partnership Board, Integration Board and other decision making forums in relation to the delivery ofintegrated care programmes and priorities.

 To ensure that both organisations vision and core values in relation to health and social care integration are actively promoted, supporting the leadership teams to make this a reality, actively contributing to the corporate management and governance across the CCG and the Council as appropriate.

 Manage and put in place robust communication arrangements of complex information relating to the monitoring and delivery of projects. This information will need to be communicated across a range of organisations and stakeholders within Brent.

 To lead on the delivery of all programme and project work streams, ensuring that they achieve their objectives and are delivered to high quality standards, within target timescales and within agreed budget.

 Review, develop, motivate and lead managers and staff to ensure work programmes and integration plans and joint commissioning arrangements are properly executed.

 Lead the production of realistic, deliverable programme plans, agreed with both organisations.

 Lead on the development of effective relationships with key stakeholders in relation to the delivery of all integration activities.

 To establish effective governance arrangements in relation to all integration programmes and priorities, which meet the requirements of both the CCG and Local Authority to provide assurance that statutory requirements are being delivered.

 To keep abreast of national policies and best practice that could impact upon integrated care; develop an understanding of key priorities and act as a source of advice to relevant partnership and integration boards on issues.

O peratio nal R esponsibi lities  Work in a matrix management style, providing leadership in the development and delivery of integration priorities across Brent, fostering positive working relationships with clinical and professional leaders and managers within both organisations.

 Establish appropriate programme structures and project work streams including processes and systems; PIDs and business cases which include objectives and outcomes, areas of project activity, criteria to evaluate success, timescales, risk logs and mitigating actions.

 Ensure coherence across the whole programme of work and maintain an appropriate working environment to support delivery of the key objectives for each integrated care project.

 Ensure robust monitoring of the overall programme of work and each of the project work streams to ensure that they are delivered according to the project plan, quality standards, budget and timescales. This will involve regular reporting to the Brent CCG QIPP PMO and the Council Change and Improvement Board.

 Ensure any deviations to integration plans are identified and explained and to investigate problems and propose solutions to improve the work of the Integration Board.

 Responsible for the development and implementation of a consultation and participation strategy to involve service users and carers in the planning and delivery of integration project work streams as required.  Prepare strategic and specific progress reports for the Integration Board, the CCG and Council and the Health and Wellbeing Board.

 Support the Brent CCG Chief Operating Officer and Brent Council Strategic Director, Adults and Community Wellbeing to deliver and oversee governance and effective management of all integration programmes, including the BCF section 75 agreement and pooled budgets and the creation, delivery and management of the Sustainability and Transformation Plan.

 Line Management responsibility for the project managers and other programme staff..

Per son Sp ecific atio n

Expe rienc e  Successful experience of working at a senior management level within either social services and health (or both).  Strong understanding of both health and social care, and demonstrable experience of working within an integration environment.  Demonstrable experience of managing large complex programmes and projects and achieving deliverables within a recognised programme management approach.  Demonstrable experience and achievement of measurable/evidence based results in the commissioning and/or delivery of health and social care services.  Demonstrable experience of effectively managing budgets and other resources including evaluating competing budget priorities to achieve deliverables within the limits agreed.  A proven track record of delivering in-depth change management particularly in relation to service redesign and innovation.  Proven experience and ability to assess risk whilst effectively developing services and strategic priorities.  Proven experience and ability to critically analyse the programme and develop innovative and practical solutions.  Demonstrable experience of delivering evidence based and measurable outcomes for service users and carers through the commissioning of services and integrated working.  Experience of analysis and interpretation of statistical data to contribute to the effective planning and selection of appropriate commissioning and procurement options.  Proven track record of working and communicating effectively with a wide range of internal and external bodies to achieve corporate and service objectives of comparable organisations.  Proven track record of working effectively within a political environment, providing clear, balanced advice and guidance on strategic issues that achieve the corporate and service objectives of the organisations.  Evidence of an understanding of the workings of health and local government, and the legal, financial and political context of public sector management. Kn ow ledg e  Sound knowledge and understanding of the role of both CCGs and local authorities.  An in-depth knowledge and understanding of whole system working and how integrated working can improve outcomes.  An in-depth practical knowledge of major legislative, social and economic issues facing commissioning and delivery of health and social care services, including knowledge and experience of NHS related issues and commissioning arrangements.

 Some knowledge of the policy context for integration, including the Better Care Fund, The Care Act, Sustainability and Transformation Plans and the Five Year Forward View.

Skil l s and Abi lities  Thinking strategically with capability to translate strategy into practical operational delivery plans.  Leadership skills which foster a positive and motivated organisational culture which helps to secure commitment and ownership.  Ability to empower, motivate and manage the team, to establish effective teams with the ability to allocate resources and undertake effective monitoring.  Ability to interpret and achieve results through influence and negotiation with a wide range of stakeholders.  High level presentational and interpersonal skills, with the ability to communicate clearly and convincingly to a wide range of audiences and local communities.  Ability to produce and present quality reports setting out information, analysis and interpretation in a clear and persuasive manner.
